Blackpool Teaching Hospitals NHS Foundation Trust is situated on the west coast of Lancashire, with services covering the local authority areas of Blackpool, Fylde and Wyre. The Trust is part of the Lancashire and South Cumbria Integrated Care System (ICS) supporting a population of around 1.6 million people.
We have three main hospitals providing acute services to around 330,000 local residents. Job overview
Blackpool Teaching Hospital is currently offering an exciting opportunity for Senior Nurse/ Therapist with Acute and Community experience to lead our Community Urgent Response Teams providing Virtual Wards, Rapid Response and Community IV service across the Fylde Coast.
You will play a pivotal role, working closely with the Integrated Community Care Directorate Tri/Quadumvirate and will take the lead on designing, implementing and driving Urgent Response Services.

Ensure service and team level performance is monitored and delivered in accordance with agreed targets and that any areas of underperformance are addressed in a timely manner and corrective action taken.
Ensure compliance with policies, procedures and clinical guidelines, and lead or participate in their development.
Establish effective professional multi-disciplinary relationships both within and outside the organisation. Participate in regional and national meetings as appropriate.
4. Provide leadership to staff within the community nursing and rehabilitation services by visibly supporting and communicating the Trust’s overall strategic vision, key objectives and corporate policies and systems.

Detailed job description and main responsibilities
The post holder will be responsible for the operational management, development and delivery of the Community Urgent Response services within the Community Directorate and for ensuring the provision of high quality, safe and responsive services within the available resources.
The post holder will work in a collaboratively with all key partners to ensure the service meets the needs of the local population and local and national targets.
