Job overview
An opportunity has arisen for a Band 4 Duty Coordinator to work within the Integrated Health and Social Care community rehabilitation team.
The role will be varied and exciting making use of both your administration and clinical skills and will be integral in deciding on the correct client pathway and hence contributing to a positive client outcome and experience.
We are looking for a dynamic motivated individual to play a vital role in supporting JCR teams to triage and prioritise referrals. You will be working closely with a variety of clinical staff and with numerous hospital departments and GP surgeries. You will be part of a supportive and friendly team which consists of physiotherapists, occupational therapists, rehab support workers and administrators.
You will interact with a wide range of partnership organisations, clients as well as their relatives and or carer.
The ideal candidate will need to be confident, proactive and have excellent communication skills and be able to work as part of the team, as well as being confident in working autonomously.

The Joint Community Rehabilitation (JCR) team is a short-term service which assesses and treats people within their own homes. The service aims to prevent unnecessary admissions to hospitals or long-term care, enable earlier discharges from hospital and promote independence and quality of life. We deliver a 7 day service and share working weekends and Bank Holidays.
As well as offering continuing professional development and clinical supervision, we also offer excellent support for those wishing to develop a special interest as well as a comprehensive in-service training schedule.
The post holders will be required to work flexibly and visit people in their own homes. The ability to travel around the county is essential.

Applicant requirements
Person specification
Qualifications
Essential criteria
- Educated to GCSE or equivalent Grade C or above, with English and Maths.
- Care Certificate
- NVQ3 / diploma equivalent
- Foundation Apprenticeship degree or Postural Stability Level 4 qualification or relevant NVQ4, or already working towards with the commitment to achieve within an agreed timeframe
Desirable criteria
- Relevant Health, Social Care or Information and advice qualification
- Further education and training courses undertaken relating to medical, orthopaedic or surgical rehabilitation
- Level 2 Health and Safety certificate
- First aid qualification
Experience
Essential criteria
- Minimum of 2 years working as a Rehabilitation Support Worker or equivalent health or social care experience.
- Experience of working within an acute healthcare setting
- Experience of working as a lone worker within a community rehabilitation setting
- Administration experience in NHS or similar complex organisation with direct contact with clients
- Experience of interprofessional working
- Experience of computerised record systems
- Experience of complex diary management
- Excellent organisational Skills and ability to work to tight deadlines
- Ability to manage a diverse workload whilst dealing with frequent interruptions
Desirable criteria
- NHS administration experience
- Experience of using an IT System and administration experience
