Job overview
An exciting opportunity has become available in the West Integrated Neighbourhood Team for a senior nurse to join our Camden Integrated Community Healthcare (CICH) workforce in part time capacity. As an experienced Senior Community Nurse working in a Camden Integrated Neighbourhood Team you will provide nursing interventions to a range of housebound patients and help to manage their care supported by a skilled team of nurses. We are proactive in looking for innovative nurses who will facilitate the delivery of excellent community nursing care, the development of new ways of working and who want to progress their career.
Following the NHS long term plan, CICH is proud to lead on putting neighbourhoods at the heart of integrated care. The service is set up to deliver health interventions within the framework of the neighbourhood model, working in partnership with other key primary care providers and the local voluntary care sector organisations. This role will be based at West Integrated Neighbourhood team at The Abbey Centre, however the post holder would be expected to work across Camden DN service according to the needs of the service.

Your role will include providing high quality care for patients in a community setting, You will work closely with the Band 7 Team Leader to review and manage a designated caseload of patients. This will include undertaking holistic assessments for complex patients, reviewing patient nursing care needs and supporting junior members of staff to undertake their role. We are looking for a Registered Nurse experienced at Band 6 level (or with substantial Band 5 clinical and managerial experience) to work in skill mixed, seven day a week nursing team. The team is supported by an overall Operational Lead in one of our five Integrated Neighbourhood Teams. We also have a range of Clinical Practice Educators and Professional Leads who are on hand for supervision and support. You will work alongside Complex Care Nurses, Occupational Therapists, Physiotherapists, rehabilitation assistants pharmacy technicians, students and a dedicated mentor; as well as colleagues within Social Care, Mental Health, voluntary Sector and General Practice.
You will have access to and support from the wider expertise of our specialist teams including Evening and Overnight Nursing team, early supported discharge team, Rapid Response, Palliative Care, Tissue Viability, Bladder & Bowel, Community Stroke and Neurology team, Respiratory, Heart Failure and Diabetes teams, the in-patient wards: who all aim to work very collaboratively. Your role will include working closely with our GP colleagues and practices.

Detailed job description and main responsibilities
Band 6 nurses work closely with the people who use our services, their families, friends and carers and play an important contribution in how people experience our services. Band 6 nurses are expected to be kind and responsive but professional and informative and contribute to the quality of the services we provide by:
- keeping the people who use our services as safe as possible through the use of sound clinical skills and effective clinical risk assessments
- ensuring the best possible clinical outcomes by using up-to-date skills and adhering to evidence based policies and procedures
- ensuring the people using our services have a good experience by respecting, empowering and working in partnership with people throughout the care planning process
We believe that the best health care is delivered by multi-disciplinary teams working well together and in partnership with other teams and services to provide seamless care. Band 6 nurses work as part of a team and you will contribute to the success of the team, drawing support from more experienced nurses and other health professionals but also by supervising junior staff and students nurses to ensure they are working to the standards which the people using our services deserve.
