The band 6 diabetes nurse vacancy is to join the diabetes nurse team working on Trafford, Wythenshawe and Withington hospitals .
It will include working closely with other members of the MDT including diabetes consultants , diabetes podiatrists, dietitians, Gp and district nurses.
The role will involve working cross sites and in the outpatient and inpatient setting. Supporting patients living with diabetes and also diabetes education will be a major part of this role.
To provide expertise in diabetes care by providing information,
support, advice and specialist knowledge to patients, their families
and carers, and to act as a resource to the multi-professional team.
To participate in the co-ordination of patient care to ensure continuity throughout the patient’s care pathway.
To contribute to review and development of the service, and to reflect the needs of the patient group.
To liaise closely with the health care teams of inpatient wards, outpatient department and primary care, including Consultant Physicians and GP’s.
To demonstrate knowledge and experience of diabetes management and clinical leadership skills. To monitor and improve standards of care through clinical audit
Deliver patient education and hold qualification as an accredited
diabetes educator (DESMOND and/or DAFNE)- or be willing to
undertake training
Develop and deliver staff education within the hospital and community setting
