In the community, we deliver services that focus on people with long term conditions living well outside hospital, through our integrated locality teams working with district and community nursing teams. Community members of staff also provide care to patients in their homes and from a number of clinics, health centres and GP surgeries.

Are you a compassionate and detail-oriented healthcare professional ready to embrace innovative care delivery? Our dynamic Virtual Ward team in East Sussex is seeking a Remote Monitoring Clinician to help transform patient experiences and outcomes from the comfort of their homes.
The Virtual Ward forms part of the Urgent Community Response (UCR) team. We are a 7-day service that supports acutely unwell patients that would otherwise be in a hospital bed, supporting both early discharge from hospital and admission avoidance. The team responds to the needs of patients in the place they call home.
The service provides both in person care from nursing, therapies and social support, as well as remote monitoring of vital signs and facilitation of remote consultations and communication with patients.
We are looking for experienced nurse, paramedic, allied health professional (AHP) with a passion for patient care and the drive, enthusiasm and talent to develop services to support patients on the Virtual Ward. You will want to develop your skills and push yourself outside the normal boundaries of practice and will have a real drive to make sure your patient gets the best treatment possible.

The post holder will be an experienced Registered Nurse, HCPC registered Paramedic or AHP who will work within the Virtual Ward team. We would encourage applications from professionals with high level clinical skills, knowledge and experience in a constantly evolving environment.
As a Band 6 Remote Monitoring Clinician you will: Provide leadership to the team on shift and support the development of the service through leading on projects and audits. Support the clinical coordination of the team on shift. Lead the onboarding and clinical management for patients on remote monitoring. Develop your clinical skills in conducting remote clinical assessments and treatment for patients across the Virtual Ward pathways. Develop new skills outside of your normal scope of practice to enable a single holistic assessment approach, following established escalation pathways.
The role will be primarily office based within the Virtual Ward Hub. At present the role is core working hours 8:00 - 16:00 / 8:00 - 18:00 with weekend/bank holiday working on a rota basis. The wider Virtual Ward service operates 7 days a week covering shifts 08:00 until 22:00.

- Teaching Qualification and/or teaching experience of more junior staff
- Relevant specialist course or equivalent post registration development / experience
