South Warwickshire NHS Foundation Trust (SWFT) provides the hospital services to South Warwickshire from four hospitals:
- Warwick Hospital
- Leamington Spa Hospital
- Stratford Hospital
- Ellen Badger Hospital
We also deliver out-of-hospital community services to the whole of Warwickshire and beyond serving a population of more than half a million from various clinics. This includes our Community Teams in North Warwickshire.
What is a Foundation Trust?
South Warwickshire University NHS Foundation Trust was formed on 1 March 2010. A foundation trust status gives us greater freedom to manage our own affairs, and makes us more directly accountable to local people through our Members, who are drawn from the local population and staff.

Probationary Period
All new staff to the Trust will be subject to a standard 6 month probationary period, the details of which will duly follow in your contract of employment.

Agile and Flexible Working
All our staff will have a work base. However the Trust operates as an Agile Working Team therefore staff may be working from home on a regular basis as required by the service. Band 6 Rapid Response Overnight Nurse
Hours: 23 hour post (x2 11.5 hours shifts 20.00-08.00)
South Warwickshire Foundation Trust, Out of Hospital Community Services in collaboration with Myton Hospice, Shakespeare Hospice and Shipston Home Nursing are working together to provide a Rapid Response overnight service within the South Warwickshire area.
The aim of the service is to provide care for patients and their families, helping them to achieve their preference to be cared for at home at the end of life. The service also offers urgent support to catheter patient in order to prevent hospital attendance. The service will operate between the hours of 20.00 and 08.00 staffed by Band 6 Registered Nurses and Senior health care assistants. The team will work closely with the existing community teams to ensure patients receive a seamless and wrap around service and excellent care.
We are looking for an experienced, skilled and knowledgeable Registered Nurse who is able to work flexibly as part of the team. If you feel you have the necessary qualifications, qualities and a passion to provide excellent end of life care we would love to hear from you.

To provide strong clinical leadership and empower team members to ensure all aspects of patient care are delivered, demonstrate clinical excellence and maintain consistently high standards of nursing practice in palliative care and catheter care.
The post holder will be responsible for actively managing a clinical caseload of patients requiring specialist core palliative care needs.
Manage support and develop Healthcare Assistants. Act as a resource for professionals and non professionals involved in palliative care within the community. Establish effective working relationships with Primary Care Health Teams to provide palliative care nursing to patients and families within their own home.
