Job overview
Due to retirement, we are seeking an enthusiastic and innovative Band 6 Nurse to join our well established and integrated Community Falls Service.
This is an excellent opportunity for those passionate about improving the lives of older people at risk of falls.
The successful applicant will further develop experience and skills in multifactorial assessment and rehabilitation and contribute to Falls Prevention education across Central Manchester.
Applicants should have a keen interest in supporting the nursing provision within our strong multi professional team of specialist Nurses, Physiotherapists, Occupational Therapists and Rehabilitation Assistants, in addition to the wider objectives of the Integrated Community Rehab Services.

Advert
The role is primarily domiciliary and requires daily travel across a number of locations throughout the day.
- Carry out multifactorial and nursing assessment of older people, many with complex presentations, in their place of residence and other community settings.
- Apply specialist knowledge in the specific area of falls and care of the older person, drawing in clinical knowledge of multimorbidity, polypharmacy and bone health.
- Use knowledge, advanced clinical reasoning and problem solving skills to analyse clinical and non clinical findings to form accurate diagnoses and prognoses in a wide range of complex conditions.
- Proactive case finding to identify hidden or future fallers and others in our population, to minimise the risk of more serious consequences, falls and injuries and to positively influence their long term health, wellbeing and independence in line with the Manchester public health and wellbeing strategy.
- Provide Falls Prevention education across Central Manchester with a range of stakeholders, including residents and family/carers. This may include talks, open days and bespoke events, group and individual input.
- Deliver a strong educational service around falls and falls management for health and social care professionals, voluntary/third sector groups, primary and secondary care.

Education / Qualifications
Essential criteria
- BSc or Diploma in Adult Nursing or equivalent
- Part 1 NMC Registered Nurse
- Evidence of current CPD including relevant clinical training or courses
Desirable criteria
- Relevant postgraduate training
- Active member of appropriate clinical interest group or network
- Nurse prescriber
- Assessor / Mentor
Experience
Essential criteria
- Previous or current NHS experience as a Registered Nurse
- Proven years experience as a Band 5 Registered Nurse
- Experience within a Falls Prevention Service or similar care of older person role
- Evidence of working effectively and sensitively in a multidisciplinary team
- Supervision and appraisal of students, registered and non-registered staff
Desirable criteria
- Evidence of NHS experience at Band 6 level
- Adult community health experience
- Use of EMIS
Skills
Essential criteria
- Developed skills in patient safety and Governance systems related to falls awareness and prevention, or adult community working
- Ability to undertake appropriate assessments in a variety of relevant settings, including recognising and responding appropriately to the deteriorating patient
- Commitment to own training and development
- Well managed self-management skills such as time management, organisation and prioritisation, and the ability to respond in a time critical way
- Ability to work as part of a multi-professional team as well as independently
- Ability to problem solve and respond to change - demonstrable flexible approach
