Due to retirement, we are seeking an enthusiastic and innovative Band 6 Nurse to join our well established and integrated Community Falls Service.

This is an excellent opportunity for those passionate about improving the lives of older people at risk of falls. 

The successful applicant will further develop experience and skills in multifactorial assessment and rehabilitation and contribute to Falls Prevention education across Central Manchester.

Applicants should have a keen interest in supporting the nursing provision within our strong multi professional team of specialist Nurses, Physiotherapists, Occupational Therapists and Rehabilitation Assistants, in addition to the wider objectives of the Integrated Community Rehab Services.

The role is primarily domiciliary and requires daily travel across a number of locations throughout the day.

• Carry out multifactorial and nursing assessment of older people, many with complex presentations, in their place of residence and other community settings.
• Apply specialist knowledge in the specific area of falls and care of the older person, drawing in clinical knowledge of multimorbidity, polypharmacy and bone health.
• Use knowledge, advanced clinical reasoning and problem solving skills to analyse clinical and non clinical findings to form accurate diagnoses and prognoses in a wide range of complex conditions.
• Proactive case finding to identify hidden or future fallers and others in our population, to minimise the risk of more serious consequences, falls and injuries and to positively influence their long term health, wellbeing and independence in line with the Manchester public health and wellbeing strategy.
• Provide Falls Prevention education across Central Manchester with a range of stakeholders, including residents and family/carers.  This may include talks, open days and bespoke events, group and individual input.
• Deliver a strong educational service around falls and falls management for health and social care professionals, voluntary/third sector groups, primary and secondary care.
