We provide a full range of acute clinical services from Northampton General Hospital and Danetre Hospital in Daventry. We are a designated cancer centre and have recently invested in a number of additional specialist services, including in-patient renal services and interventional cardiology. All elective inpatients and emergency admissions are screened for MRSA. As part of our work to improve clinical outcomes we are taking part in a national patient safety programme and have invested in systems to capture patients views on the service we provide. We understand that sometimes a small change can make a big difference for patients.

An exciting opportunity has arisen for an experienced health care worker who holds a Level 4 Vocational qualification to support the Macmillan Rarer Tumour Nurse Specialist, providing high quality patient centred care to those referred with a suspicion of a rarer cancer
The post holder will act as an assistant to the Macmillan Rarer Tumour Nurse Specialist, undertaking an assessment of patient/carers holistic needs, providing general information help and support from the point of diagnosis and throughout the cancer pathway.
The post holder will be the first point of contact for patients/carers, identifying and addressing their general holistic needs, signposting to other services/agencies, alerting the Macmillan Clinical Nurse Specialist when patients require specialist input.
The post holder will also support the patient pathway by chasing and bookings appointments and provide holistic support for patients who attend regularly for lifelong treatment who do not require specialist support
The role will be required to undertake be some administrative aspects to support the service including data entry onto the Somerset Cancer Registry

We welcome applications from caring, motivated Health/Social Care Workers who have experience working with cancer patients. Educated to GCSE level in English and Mathematics. Must possess a vocational qualification at level 4 to be considered for this post and hold or be willing to undertake the Foundations of Cancer Care or equivalent in line with the National Cancer Education Framework
Interested applicants must also be able to demonstrate good communication, interpersonal, organisational and IT skills.
To work under the direction of the registered nurse providing high quality care to patients and carers living with a rarer cancer
The role involves being responsible for aspects of direct care delivery, assessment of patient and carer needs and the provision of general information, help and support.
To be an extended member of site-specific MDTs, ensuring effective communication across the Trust, Primary Care and with statuary and voluntary agencies
To undertake the administrative aspects of the service, coordinating the patient pathway between organisations

Detailed job description and main responsibilities
Contribute to the holistic needs assessment and the development of an individual care plan under the direction of the registered nurse
Implement, monitor and review aspects of the care plan with the patient and carer, reporting any changes in the patient’s condition to the registered nurse
Evaluate the outcome of care delivery with the registered nurse
To review patients on the ward under the direction of the registered nurse
Demonstrate the ability to recognise and respond appropriately when faced with a sudden deterioration or an emergency, alerting the team or enabling rapid response as appropriate.
To triage incoming calls, providing general advice to healthcare professionals and patients/carers/, referring complex/specialist issues to the Macmillan Rarer Clinical Nurse Specialist
Provide general telephone advice, signposting patients/carers to other resources as appropriate.
Coordinate appointments or investigations to fast-track patients through the system.
Liaise with the MDT coordinators to ensure a seamless service for patients and smooth transition through the patient pathway.
To identify and address patient/carer general information needs
To support patients/carers in accessing the Northants Cancer Guide, navigating the system to address their holistic needs
To proactively telephone patients to assess on-going/change of individual needs to enable prompt assessment and management of worries/concerns
Support the registered nurse in the coordinate of care to facilitate safe and effective transition between services to provide seamless support for people living with cancer.
Support the delivery of patient and carer training and education, including the use of equipment.
Advise patients on individual self-care management strategies and education as necessary, including how to self-assess.
Encourage and support healthy lifestyle choices
Provide Level 1 psychological support to patients/carers, referring patients who require Level 2 support to the registered nurse.
Contribute to the development of the specialist nursing service
Comment on proposed changes to policies and procedures to enhance the specialist nursing service
To support the registered nurse in the development and implementation of support groups
To support the development and initiation of end of treatment summaries, ensuring patients receive a summary at the end of each episode of treatment
To be a flexible member of the team able to respond to the changing needs of patients and the service
To provide a vital link between patients and the registered nurse, highlighting patients that require specialist assessment.
To attend the tumour site MDT in the absence of the registered nurse
To take responsibility for the administrative aspects of the nursing service
To review patients in outpatient settings under the direction of the registered nurse
To support the registered nurse in responding to EAU alerts for patients admitted as an emergency
To order equipment and supplies
Understand that there will be frequent exposure to distressing situations and when to seek appropriate support/advice
Attend monthly/quarterly meetings with the registered nurse/Cancer Lead Nurse to review the effectiveness of the role
To achieve the agreed competencies to fulfil the role with 6 months of being in post
Have knowledge of the full range of resources and services available through Macmillan cancer Support and The Macmillan Information Centre
Demonstrate an awareness of the limits of own practice/knowledge and when to seek appropriate support/advice
To participate in the formal review process and plan future personal development and competencies to be achieved
Document outcomes/consultations on the Somerset Cancer Registry, ensuring effective communication with all members of the cancer multidisciplinary team
To utilise IT systems to inform decision making
To support and contribute to clinical audit and research initiatives
