Trafford Specialist Palliative Care Team
The Associate Macmillan Palliative Care Nurse will work as a member
of the wider locality based multidisciplinary Macmillan Palliative Care
Supportive Service. The team is very supportive and a full
competency-based induction will be offered, supported by a Band 7
Clinical Nurse Specialist.
The post holder will support Neighborhood teams to deliver locally driven high quality care, responsive to patient and carers’ needs. The focus of the multidisciplinary approach will be to support patients and carers to live well, prevent inappropriate hospital admission and meet their preferred priorities for care.

Up to date registration with the NMC
Health Related Degree or equivalent portfolio experience
Experience evidenced by completion of specialist short courses, post-graduate training, etc. or relevant
Relevant post registration experience
Experience in delivering palliative care.
Experience of working collaboratively
Experience of working within a multi-disciplinary team
Experience of working in a community setting.
Experience of teaching training others
Experience of supporting and supervising other staff in the clinical environment
Knowledge of a broad range of assessment tools relevant to cancer/palliative care.
Demonstrate the application of evidence to work in palliative care Advanced communication skills or willing to undertake 3 day National Advanced Communication Skills training
Understanding of the Specialist Palliative Care Nurse role and roles of other professionals (relevant to the client group)
Knowledge of the principles of Clinical Governance/Audit
In depth knowledge of National Policies and Procedures (relevant to the specialist client group)
Good presentation skills both written and verbal
Good caseload management skills including experience of managing own caseload
Car Driver/access to a car for work purposes
