Job overview
An exciting opportunity has arisen for an experienced Diabetes Specialist Nurse to join our existing team, providing Adult Diabetes Services across the Havering area for two days per week.
Our Integrated service model encompasses a multidisciplinary approach to Diabetes Care in the community setting, allowing shared responsibility for the person with diabetes. The service aims to maximise the opportunity for preventing complications of diabetes through an effective empowerment and partnership approach, focusing on individual care planning, support for professional and patient education and emotional and psychological care.

The main responsibilities of the post:
The successful candidate would be expected to work within a team of skilled diabetes specialist nurses and to deliver a high standard of evidence based clinical care to patients and their carers, and to work as part of the Integrated Diabetes Service, within the community setting. The successful candidate must be confident to deliver education sessions to both patients and healthcare professionals.
The successful candidate will require excellent communication skills and will need to be confident to work alone and within a multidisciplinary team.
We are seeking a highly motivated individual who will contribute to the developments of our multi-disciplinary specialist community service and who has a commitment to continued personal development and is able to travel efficiently across the area of Havering , including to patients in their own homes.

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex
High Cost Area Supplement – London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and a maximum payment of £5,735 per annum pro rata).
