Job overview
· Work as a member of the multi professional team providing clinical care to patients with a diagnosis of learning disability from the age of 18years and above as delegated by the Registered Nurse.
· Ensure a high standard of patient centred care.
· Promote self- management of health needs and adopt a positive risk based care approach.
· Complete relevant forms of care, under the direction of a Registered Nurse.
· Nursing Associates can assist in the delivery of care to patients, with a multiple combination of multiple chronic conditions, mental health issues, medication related problems and social vulnerability (defined as complex care).
Advert
- To act as lead clinician those service users identified and directed by senior practitioner assessing service user’s needs, developing and implementing care plans, including risk assessments and carers assesments.to participate in induction and support of new staff and students and to co-supervise junior staff.
- Support patients with all elements of the nursing process and be able to assist the team in the on-going assessment, planning, management and evaluation of care.
- To plan and organise own activity, prioritising workload appropriately in order to make the best use of time and feeding back to the team as appropriate.
- To input all activity with service users onto RIO or equivalent data in order to enable the collection of accurate statistics, as required by the Trust.
- Recognise issues relating to safeguarding vulnerable children and adults to report any problems or raise concerns to appropriate registered care professionals.
- Demonstrate understanding of caring for patients with particular conditions for example mental illness, learning disabilities and autism.
- Signposting patients to services which may be of benefit to their wellbeing, promoting independence and capability with their own network and community.
- Signposting patients to services which may benefit of their wellbeing, promoting community connections and make the best use of local asset.
- To administer medications in accordance and with NMC code and Trust policies.

Detailed job description and main responsibilities
· To ensure that individual needs are identified through holistic assessments and that needs are met through the planning, implementation and evaluation of appropriate packages of care.
· To work collaboratively with multi-disciplinary, multi-agency colleagues to provide a co-ordinated person-centred service, providing equitable health outcomes.
· To work autonomously in the community with responsibility for own caseload, seeking clinical supervision as required.
· To work as part of a multi-disciplinary team providing 24hr assessment and treatment service.
· To be innovative in providing care through interpreting complex needs problems and challenges.
- To assist in the provision of the highly specialist/complex assessments for patients with neurological and physical disability who also have a learning disability
We provide a Multi-professional preceptorship programme that is aligned to the National Preceptorship Framework for Nursing and have been awarded the Interim quality mark from NHS England. Our programme is provided to all Newly Qualified; Nurses, Nursing Associates, Internationally Educated Practitioners, Allied Health Professionals and Return to Practice Practitioners. In addition to the programme we also offer all preceptees the opportunity of enrolling and completing the level 7 (Masters) 15 credit validated module “Preceptorship for Healthcare” (AP7026).
Newly qualified Social Workers are offered the Assessed and Supported Year in Employment programme (ASYE) which is similar to a preceptorship programme.
