This post forms part of the Continuing Healthcare (CHC) team which provides a single point of contact for all Continuing Healthcare referrals. The key function of this post is to assess patients who are referred to the service, in consideration of Funded Nursing Care and Continuing Healthcare. The role will involve assessing clients against the Continuing Healthcare Checklist and Funded Nursing Care review document, considering eligibility for full Continuing Healthcare assessment, reviewing suitability of care packages and ensuring quality standards of care and documented evidence by care providers. The role is undertaken under the supervision of the Nurse Assessors and Senior Nurse Assessors.

•Undertake completion of Continuing Healthcare Checklists and annual reviews of eligibility for Funded Nursing Care as defined under the National Guidance for Funded NHS Nursing Care and The National Framework for Continuing Healthcare (October 2018).
•Recognise those clients with a change in care need, and refer to a senior for further assessment of eligibility.
•Ensure that all relevant care plans, risk assessments and care records reflect the needs of the client, are up to date, completed to an approved quality standard. Where these are missing or lacking in quality, you will provide feedback to the care provider to ensure this is resolved.
•Liaise with members of the client’s wider MDT to ensure reviews are undertaken with all relevant assessments and updates from those involved, and that these are obtained before a review is undertaken
•You will follow up with care providers to monitor any improvements you have requested, and where these are not completed within the requested timeframe, you will escalate this within the Continuing Healthcare Team.
•Report to providers and escalate any concerns or quality issues to the Safeguarding Lead or Quality Assurance Manager.

Duties and Responsibilities
•Work with the appropriate quality and safeguarding leads to identify and manage concerns relating to patient safety and quality issues identified through team assessment or review.
•Under supervision, participate in the completion of initial Continuing Healthcare multidisciplinary assessments and recommendations for eligibility.
•Review eligibility and appropriateness of the commissioned care arrangement against the patients’ needs.
•Ensure good time management and organisational skills so all reviews are undertaken within the required timeframe and in adherence with local and national quality premiums.
•Ensure all written reports, documents and assessments are produced to a high standard and in line with the NMC directive.
•Monitor the safety and quality of the care provided to patients.
•Provide advice to key stakeholders, clients and their representatives around FNC/CHC processes and requirement, supporting training and education of providers and their staff so that they understand the principals of the relevant guidance, framework and legislation.
•Positively engage with the FNC/CHC quality assurance process, taking into consideration the feedback provided to ensure that this informs and shapes ongoing learning and development within the role.
•To take part in training programmes which support ongoing pertinent professional skills and knowledge develop.
•To undertake organisational and professional mandatory training.
•To act as patient advocate to support vulnerable adults and where necessary identify and report any situations of adult abuse immediately to their line manager and commissioners
•Deliver, where appropriate, sensitive and confidential information to clients, their representatives and key stakeholders about funding decisions and changes to care requirements.
•Comply with all Standard Operating procedures, service and organisation frameworks and policies
•Be professionally and legally accountable and responsible for all aspects of own work, including the management of patients and assessments, while contributing to quality improvement of the service
•Be actively involved in the audits undertaken by the service, ensuring recommendations are implemented and making suggestions/recommendations on the findings
