Hi, I’m Angela and I am the Clinical Lead for the cardiac service. We are looking for a Cardiac Nurse who has experience in looking after patients with heart failure to join our established and experienced community service. The post holder will work within a highly skilled team to support and manage patients with complex Heart Failure, using evidence-based practice and patient centred goals to assess, plan, implement and evaluate interventions.

• To Manage a caseload of patients with complex Heart Failure, using evidence -based practice and patient centred goals to assess, plan, implement and evaluate interventions
• To work as part of the multidisciplinary team with the aim of preventing admission, supporting effective discharge and providing ongoing care for patients with Chronic Heart Failure within the community
• To provide expert support, advice, and education to raise awareness of the need of the patient with Chronic Heart Failure
• To Undertake all aspects of clinical duties as an autonomous practitioner with access to advice and support from senior colleagues
• The post holder will be an active team participant within the service and provide in-depth clinical knowledge.
