The post holder will work in the Children and Young People’s Continuing Care Team, undertaking assessments and reviews to assess whether a child’s needs are such that they meet the criteria for full NHS continuing care. The post holder will be expected to undertake NHS Continuing Care assessments, comprehensive nursing assessments, contribute to the completion of EHC plans and support during multidisciplinary team meetings when completing Continuing Care documentation. The post holder will provide professional, clinical, and subject matter expertise on matters relating to children’s Continuing Care processes for the ICB, Local Authority, healthcare providers and other local agencies and organisations.
The post holder will be responsible for undertaking assessments and reviews of individual’s eligibility for Continuing Care and case manage existing patients. The post holder will ensure that assessments and reviews are carried out in a timely manner according to national specifications.
Using the Children’s National Framework for Continuing Care, work in conjunction with acute hospitals, community services, social workers and other professionals to complete joint health and social care assessments and care plans.
Gather, evaluate and interpret relevant information about children’s health from all relevant professionals and services.
Make recommendations to Panels regarding funding and services required by children with complex health needs and their families.
Work with the Senior Nurse Assessor to procure care packages on a case-by-case basis. Develop care packages for children with complex health needs, including Personal Health Budgets.
To ensure Children’s Continuing Care service provision is clinically appropriate and delivered to maximise children/young people’s safety by meeting their ongoing complex needs

Duties and Responsibilities
Demonstrating skilled communication skills, to work effectively with different stakeholders including young people and their families, social services colleagues, provider organisations and other professionals
Educate, inform and involve children and young people and their families, carers and professionals on matters of local health service provision
Maintain appropriate written and electronic records and communicate information as required with all relevant parties whilst maintaining appropriate confidentiality of the patient record according to governance requirements
To monitor agreed standards of care, provide a person-centred service using best practice, knowledge, and act as an advocate for children and young people with complex care needs where appropriate
The post holder will be expected to manage their own caseload with support from the Senior Nurse Assessor.
To participate in ongoing professional development and clinical supervision on a regular basis as well as undertaking training as necessary in line with the post.
To participate fully in the development review (appraisal) process and undertake Continuing Professional Development as required.
To adhere to the Nursing and Midwifery Council or Health & Care Professions Council Code of Professional Conduct.
Make recommendations to senior managers on policy and service development issues affecting the wellbeing of children with complex health needs.
To undertake such other duties as may be reasonably required from time to time as are consistent with the responsibility and scale of the post.
