Job overview
We have an exciting opportunity for a highly motivated health care assistant to work across our TB and Respiratory Services within the community.
The role will provide support to patients under the care of qualified clinicians supporting them to live well in the community and achieve good health outcomes. You will be expected to support the current staff in post with TB screening, pulmonary rehab and the delivery of care in both community and clinic settings.
Development through the Nursing associate pathway will be considered for the right candidate.

Provide advice and instruction to patients to explain any procedures being undertaken.
Communicate with practice staff and GP’s regarding results and collection of specimens.
Refer patient queries to the nursing staff in a timely fashion.
Report any change or deviation from planned care package to nursing staff.
Recognise abnormal results following monitoring and inform nursing staff.
Undertake the delivery of patient care packages under the supervision of nursing staff.
Manage and prioritise workload.

Be able to implement packages of care, therapy or treatment prescribed by qualified members of the team. This will initially be done with supervision from a qualified member of the team.

Detailed job description and main responsibilities
1. Provide advice and instruction to patients to explain any procedures being undertaken.
2. Understand the principles of data protection and patient confidentiality and the policies and procedures surrounding them.
3. Attend and participate in staff meetings.
4. Basic understanding of IT and ability to use e-mail and willingness to undertake training for any systems used to collect and manage patient data.
5. Communicate with practice staff and GP’s regarding results and collection of specimens.
6. Refer patient queries to the nursing staff in a timely fashion.
7. Provide understanding and support to patients having difficulty coping with their condition.
8. To be supportive and communicative with patients & colleagues.
9. To ensure at all times patients are treated with care and compassion.
10. Report any change or deviation from planned care package to nursing staff.
11. Recognise change in patients need and with instruction adapt care package accordingly.
12. Recognise abnormal results following monitoring and inform nursing staff.
13. Undertake the delivery of patient care packages under the supervision of nursing staff.

15. To liaise with other health professionals, social services, GP’s, hospital staff and other agencies involved in the care of each patient as required.

27. To provide reassurance & psychological support to patients in an acute phase of illness which may involve short episodes of intensive input to prevent hospital admission.
28. To maintain a neat and tidy appearance and professional approach to work, patients, their families & other colleagues.
29. To act in a manner that is courteous, polite friendly and considerate at all times.
30. Ensure all electronic and paper based records are kept up to date and filed appropriately.
31. To maintain accurate and comprehensive records, recording and providing statistical information by means of computer and other methods.
32. To supply information to the client where appropriate using computer database and written materials.
33. To monitor stock in partnership with the nursing team.
