Detailed job description and main responsibilities
Clinical Responsibilities This post has responsibility for patients managed under the Acute Care Hub in each of its three areas, supported by a junior doctor team. The Acute Care Physician (ACP) consultants run the Hub out of hours in conjunction with the speciality physicians (SCP).
The Acute Medical Unit (AMU) and parts of Same day Emergency Care (SDEC) Unit are the area for assessing patients by the medical team during the medical take. The medical on-call receives about 35 patients with decision to admit in 24 hours. It is supported by the consultant Physician of the Day (POD) made up from a combination of ACP and SCP. Junior doctors rotate through the medical on calls and there is a day shift of 1x FY1, 1x FY2+ (FY2, IMT or GPVTS) and 1x ST3+ all day with additional FY2+ and ST3+ on a late shift. The AMU is run Mon-Fri by the acute medicine senior and junior team. Overnight the AMU is mainly run by a FY2+ and ST3+ with an FY1 and IMT/GPVTS covering the wards (supported as needed).
The Ambulatory Emergency Care Unit (AEC) was opened in November 2014 and has nursing cover 08.00-20.30 with an ACP consultant and junior doctor covering 09.00-20.00. IMT’s are extending doctor cover 9-17:00 on weekends. This includes running a “virtual ward” and some medical day case patients.
The acute medical team also has a flexible shift mainly covering medical front door outlier patients to ensure daily senior review.
At the weekend we have a front door team covering medical admissions and AMU, a respiratory team, a gastroenterology team and a ward cover team. The weekend ward cover team which has a list of patients admitted Fri\Sat\Sun to ensure any deterioration in these patients is detected over the weekend where the speciality teams are not present. This post will cover weekend duties covering the front door with AMU and wards in a 1:8 rotation.
For bank holidays (Monday) including Easter (Friday & Monday) the Acute Care Physicians provide cover for the AECU and AMU and a day off in lieu is provided. The speciality teams provide ward cover and POD cover. Christmas has a separate rota.
Medical Education and Training The post will have a commitment to the education of junior staff and medical students, formally and informally. Supervision of junior staff will take a big part in daily clinical work. Regular departmental educational meetings are taking place.
Clinical Audit and Clinical Governance Participate in clinical audit across the department in line with Trust policy and guidelines. Implement audit and research findings across the department. Comply with the Trust clinical governance processes in a timely manner There are monthly SDU CG and mortality and morbidity meetings that are expected to be attended.
Continuing Professional Development Supporting Professional Activities are undertaken by all Consultants in a collaborative manner based on SDU needs. The candidate is expected to commit to revalidation.
Job plan
We operate an annualised job plan to accommodate flexible planning of shifts. Shifts are selected and then agreed in team rota meeting every 3-6 months.

EXAMPLE DRAFT JOB PLAN
There are six leadership roles within Acute Medicine which will be shared out between the consultants; these could potentially be taken over by a specialty doctor in the future and add to the PA count described above.

The weekly timetables are representative of the post but the fine detail will change depending on the speciality desires of the successful candidate and the number of consultants appointed at the time. Further developments can be expected.
The post holder will participate in the weekend on call rota for acute medicine including overnight on-call. The frequency is 1 in 8 weekends. The post holder is expected to be on site 8 AM till 8 PM on Saturday and Sunday plus offsite cover overnight on Saturday when covering as POD1 (front door), and 8 AM till 4 PM on Saturday and Sunday when covering as POD2 (ward support). The post attracts category A 3% & 1.05 PA for on-call plus day off post POD 1 or 2 weekends. A day off in lieu will be given around a weekend worked (excluding any locum shifts).
Acute physicians cover the POD duties Mon-Fri 8-17 hours, the specialty physicians over the evening POD shift and overnight on-call.
