Are you a GPhC registered pharmacist looking for a career opportunity in clinical services?
We are recruiting for a Clinical Services Pharmacist to join our multi-disciplinary team at the Warneford Hospital in Oxford on a 12-month fixed term contract. We can offer a variety of flexible working solutions, including full, part-time, and annualised hour contracts.
The Oxford Health Pharmacy & Medicines Management service employs approximately 80 staff, predominantly clinical pharmacists, and technicians, supported by dispensary and administrative staff. Clinical pharmacists and medicines management technicians are based (primarily) on clinical sites, with supply and support functions at hubs in Oxfordshire.
We provides services to our mental health in-patient and outpatient teams, including Acute adult mental health services, Forensic services, Learning disability services and CAMHS, as well as community hospitals.
Your role will primarily be focussed on supporting the adult and older adult mental health teams based at the Warneford site, though you'll be working alongside clinical pharmacy colleagues within the Oxfordshire, Wiltshire and Swindon areas of the Mental Health Directorate so the ability to travel between sites will be required.

- Responsible for the development of strong clinical relationships with the patients, carers and healthcare professionals in your in-patient and community settings.
- Directly support patients, prescribers, and other healthcare professionals via attendance at multi-disciplinary team (MDT) meetings and one-to-one patient consultations, carry out medication reviews, engage in discharge-planning with the MDT and ensure the correct application of national and local guidance with regard to therapies involving medicines.
- Support and work alongside our centralised hub pharmacist where necessary (based at the Clinical Pharmacy Support Unit in Oxford).
- Support the ongoing development of the community based pharmacy services.
- Support the Trust and departmental clinical effectiveness initiatives.
- Be a member of the on-call pharmacist out of hours team as per rota where applicable.
- Be encouraged and supported to professionally develop in line with the departmental and your own personal needs as appropriate.
- Be supported in continuing professional development through regular clinical and peer group supervision and education sessions within the Pharmacy department and externally where practicable.

Detailed job description and main responsibilities
- Optimise drug therapy for individual patients by participation in MDT meetings, drug history taking and Pharmaceutical Care Planning.
- Ensure objective monitoring of clinical response and side effects of medication as part of care planning
- Ensure that prescribing and administration of medication adheres to best evidence and practice and to Trust policy.
- Promote multi-disciplinary training regarding medication.
- Provide coordinated medicine information to patients as part of the discharge-planning process.
- Liaise with members of clinical team and external agencies as part of the CPA and discharge planning and to undertake needs assessment for individual patients e.g., compliance aid requirements
- Document clinical pharmacy contacts, interventions, and records such as drug histories and treatment plans on the pharmacy system and trust EPR system.
- Provide post-discharge care to patients, carers, and relatives as part of the support network, and to act as point of contact for CPNs, GPs and community pharmacists regarding interface issues
- Provide medicines information and advice to patients, carers, hospital, and community staff.
