We are delighted to offer this Static Senior Band 6 Neuro-Physiotherapy role in our friendly and dynamic Stroke & Neuro Early Supported Discharge (ESD) Team. This post is fixed term for 12 months, to cover maternity leave. The successful candidate will be able to develop their community neuro-therapy skills and knowledge, and there will be many opportunities for training and other exciting service developments.
Base Location: Base changing to Canford House, Discovery Court, Wallisdown in Summer 2025. Working across Bournemouth and Poole sites also, and community base withing the Bournemouth, Poole, Christchurch and East Dorset locations.

To provide an efficient and effective Physiotherapy 7 day service within the Stroke and Neuro ESD Team to community patients with stroke and other neurological impairments .
To assess patient’s level of function, set realistic goals, and implement timely Physiotherapy treatment programmes with the aim of improving independence, function, and quality of life. ·
To develop specialist skills in all aspects of rehabilitation · To maintain a flexible, patient centred approach to service delivery at all times.
To assist with the safe and satisfactory discharge of patients as necessary, identifying recovery and support plans using appropriate community services and discharge to assess pathways
To ensure a positive impact on patient flow within the Trust including triaging patients and updating electronic databases in order to manage the ESD caseload
To be involved in student education and be a Practice Placement Educator as required

We are in the process of changing main staff base to Canford House, Discovery Court, Wallisdown in approximately June/July 2025 and current staff will have a 30 day consultation process. We will also work across Bournemouth and Poole sites.
Please note if the role or service is relocated as part of a planned move, employees will not be eligible for excess
mileage reimbursement. Travel from the postholder's home to the new work base will be considered a normal
commute. Any other changes not related to this relocation will be managed according to the relevant Trust or National
terms and conditions.

To complete neurological assessments in the patient’s home environment and plan holistic patient-centred neurological therapeutic input which is regularly monitored and updated.
To provide specialist neurological Physiotherapy input to ESD patients in the community, including assessment and rehabilitation input to support recovery in areas such as: physical function, fatigue, pain management, cardiovascular fitness, mood/anxiety/adjustment, prevention and management of complications, provision of basic equipment to support independence, manual handling, activities of daily living, return to work, and driving.
To provide education and support to patients, carers and families.
To assist the Stroke and Neuro inpatient therapists with discharge planning as necessary. This may include attendance at appropriate stroke update meetings (SUM) and through triaging. Collaborative working with in-patient therapists and joint discharge home visits may be required for those more complex patients.
To assist the ESD Team Lead with data collection, SSNAP, audits and reports so that the service can be monitored and developed.
To assist with neurological education to therapy assistants, peers, students on placement, the wider team and external agencies. To be responsible for attaining and maintaining own neurological competencies.
