Job overview
Full administrative support to the Local Cancer Multidisciplinary Team Meetings and proactive monitoring and tracking of the cancer patients ensuring that their journey is processed in a timely and efficient manner, in line with Cancer Waiting Times. Working closely with other hospitals departments to ensure appointments and investigations are arranged in a timely fashion.

Patient Pathway
Ensure diagnostic investigations are carried out in a timely manner and the results of such are made available as soon as possible to clinicians.
Be responsible for identifying patients coming into the Trust referred either via the fast track or other routes (at positive histology)
Track and guide (virtually) the patient through the hospital system ensuring there are no system delays which will prevent the patient achieving the cancer waiting time targets.
Be responsible for identifying and resolving delays in the patient pathway. Where this is not possible, ways forward to be discussed and agreed with the Divisional Manager and Deputy Cancer Manager.
Ensure all data in relation to the patient journey is input onto SCR (Somerset Cancer Register) which is the Cancer Services database.
Highlight any regular system bottlenecks and contribute ideas to resolve these.
Work closely with the Deputy Cancer Manager and Service Improvement Facilitator to continually improve the patient journey.
Be able to identify and advise clinical teams on when adjustments can be made to the patient journey, ensuring these are documented on the PTL and the Cancer Database –SCR

Detailed job description and main responsibilities
MDT Support
Provide full administrative support for cancer sites, local Multidisciplinary team meeting (MDT).
On advice from the Consultants and Clinical Nurse Specialists, produce a list of patients to be discussed at the weekly MDT, including all necessary clinical information such as Radiology and Pathology results for the patient that is required for the MDT discussion.
Ensure case notes, and results are available at MDT meetings. This will involve liaising closely with Radiology and Pathology to ensure reports are available in time for the MDT meeting deadline.
Liaise with Consultant Secretaries to ensure that all patients are identified and included on the MDT list for discussion
Distribute the MDT lists by a given deadline to all core members of the team
Attend the MDT meeting and take and record the minutes of the discussion live within the MDT.
Operate the Video Conferencing equipment live within the MDT
Following each MDT Meeting, undertake follow-up actions such as; GP’s notification of a diagnosis, make patient appointments in conjunction with the appointments office, ensure any investigations required are requested and forms sent to the necessary department within an agreed timescale.
To maintain an attendance register and updated list of MDT membership.
Ensure Cancer Waiting Time Targets are fully integrated into the function of the MDT meeting and that Clinicians have a full understanding of what is required in relation to the targets.
Provide Consultants and MDT members with dates of when patients should be treated by, to adhere to the Cancer Waiting Times 31 day and 62 day treatment targets.
Collect clinical information at the MDT such as staging information and pathology.
PTL
Attend a local weekly PTL with nominated cancer leads within each division to discuss all patients on a cancer pathway in the system to ensure all patients are being tracked without any delays and provide assurance that no breaches are occurring
Clinical Data
Ensure all available clinical data is collected throughout the patient pathway. Where gaps are identified, backfill with the correct clinical information
