We are seeking a new team member to join our patient-centred, progressive team and support us to deliver high quality Podiatry to our local population in the London Borough of Ealing.
Due to increased demands and more complex community health needs, an opportunity has arisen for a newly qualified/Band 5 Podiatrist to join our team.
The post holder will be working to HCPC and CNWL Trust standards and values. As a Band 5 Podiatrist you will provide high-quality care to a diverse range of podiatry patients, with varying co-morbidities, in a range of settings. You will undertake a clinical rotation as part of your induction and will be supported by annual appraisal, regular supervision and direct access to your line manager.
On satisfactory completion of an agreed set of competencies and trust preceptorship, the post holder will be eligible to progress to a Band 6 post via the trust development process. We anticipate this taking between 12-18 months, depending on the applicant. These posts will then be incorporated into our current, Band 6 Rotation, programme, and there may be an opportunity to link this rotation with other health professionals. Although fully supported, if the applicant is unable to complete the competencies within the period of 18 months, they will remain at a Band 5.

The successful applicant must be motivated to learn and develop due to the increased complexities of patients but will be fully supported on this through clinical and management supervision.
The role will primarily be treating patients with moderate/high risk/active foot needs as well as supporting nail surgery and MSK/biomechanics clinics in adults and children.
The post holder will work independently in a variety of community settings, providing a high standard of quality clinical care showing the knowledge and skills needed to meet the competencies for a band 6 Podiatrist.
You will assess, diagnose, develop and implement individualised care plans for patients with a wide variety of clinical needs, including but not limited to acute and chronic complex long term conditions (LTC), chronic diseases, vascular and wound management.
You will autonomously assess, manage, treat, educate, train others and help create pathways for complex caseloads with the Specialist leads, consisting of and including patients with: acute and chronic diabetic/non diabetic foot problems, pre and post-operative amputations wounds and acute and chronic neuropathic, vascular foot/lower limb problems, rheumatological problems, immunological problems, dermatological problems, neurological problems and mental health problems using appropriate diagnostic techniques, knowledge, skills and tools.

Clinical & Operational
- To autonomously assess and diagnose patients with a diverse range of medical and podiatric conditions, using appropriate diagnostic techniques, skills and tools underpinning all core Podiatry skills. You will be expected to ask for help if you need it.
- To take direct referrals from Primary care, community and acute settings and providing rapid access and expert clinical advice and intervention based on agreed referral criteria.
- To take hospital referrals from consultants/doctors, nurses and accident and emergency based on agreed criteria and ensuring patients at not put at risk.
- To diagnose and treat complex Podiatric disorders for both podiatric case management and in context of wider Long Term Conditions care plans.
- To perform complex wound care on patients considering their “Risk” status and to apply and practice principles of wound management using wound classification systems.
- To provide guidance and support to podiatry department packages of care; to develop and embed the need for recognising symptoms in rapidly progressing conditions, fast tracking to MDFT/specialist acute teams into daily practice for all podiatrists.
- To have highly developed physical skills, precision hand/eye co-ordination which involves highly skilled debridement techniques of acute/chronic, neuropathic and ischaemic wounds, often for prolonged periods.
- To take referrals from doctors, nurses and allied health professionals in community and secondary care based on agreed criteria and ensuring patients are not put at risk.
- To work in close collaboration with other specialist teams in all care settings to ensure a holistic approach to the care management plan of the patient is met.
- To provide emergency treatment for any lesion that is deemed to be urgent and to organise follow–up appointments for the treatment of that lesion as necessary.
- To have the ability to cope with unpredictable work patterns as necessitated in acute and high risk wound care clinics.
- To refer on urgent or exceptional cases to other healthcare professionals and communicate (in writing) with other health or social care colleagues to ensure the patient receives other services/care required.
- To motivate administration and clerical personnel to co-operate effectively with the team assisting them in identifying and helping solve problems in order to provide an efficient service.
- To lead in the planning of care packages jointly between hospital and community care providers utilising extensive skills, such as, the application of bespoke FFOs like the ‘air cast boot’ and ‘Total Contact Cast’ in order to maintain the patient’s mobility where applicable.
- To provide guidance and support to podiatry department packages of care; which includes nail surgery, biomechanics and wound care/routine care into daily practice for all podiatrists.
- To deliver specialist care in a range of settings. (for example and if available, hospital in/out-patient, ambulatory care, Community Clinics and Health Centres, Domiciliary at patient’s home and care/nursing homes).
- To deliver NICE and local commissioner specification compliant care and ensuring all aspects of your intervention are of a high clinical standard.
- To support the formulation of a foot screening strategy and implement an education and screening programme in accordance with the National Service Framework for Diabetes standards on foot care and NICE Guidance.
