Essex Partnership University NHS Foundation Trust (EPUT) was formed on 1 April 2017 following the merger of North Essex Partnership University NHS Foundation Trust (NEP) and South Essex Partnership University NHS Foundation Trust (SEPT).

Following the establishment of a new Band 7 role in the Cumberlege Intermediate Care Centre – our 22-bedded Community Physical Rehabilitation unit.
We are in the exciting position to formalise this post on a permanent basis.
We are looking for a passionate, dynamic individual who is self-motivated and looking for a challenge. If you are an experienced nurse with excellent communication skills and have extensive knowledge and skills at a Band 6 level, this may be the challenge for you.
Having established this role within the team your responsibility will be to continue this clinical leadership working alongside an experienced therapy team with other Band 7 members of staff. You will have direct influence on patient care within this ever-changing environment.

The post holder is responsible for the proactive leadership and management for all aspects of care provision within this 22 bedded Physical Rehabilitation Unit based at the Cumberlege Intermediate Care Centre (CICC) providing visible and professional support to ensure the delivery of high quality patient care and rehabilitation. This role provides leadership of this multidisciplinary team with the nursing team working in partnership with therapist, GP’s and facilities staff.

KEY RESPONSIBILITIES
- Responsibility for Patient Care
- Ensure the correct therapeutic environment is maintained in order to support active rehabilitation.
- To oversee care provision and ensure that acute records are made and maintained.
- Supervisee and monitor the clinical work of staff to ensure a high standard of patient care is maintained at all times
- As part of the MDT ensure the full health and social care needs of the patients are assessed and appropriate action initiated as necessary
- Ensure there is good communication within the MDT to ensure that patients experience a seamless service into, during and out of the unit
- Communicate with empathy and sensitivity to sometimes distressed and angry families and relatives and take appropriate action to achieve satisfactory and appropriate outcomes.
- Be part of the clinical rota undertaking shifts to lead patient care and be responsible for working with the team to seek and improve the quality of the services provided.
- In co-ordination with the Matron for CICC report and investigate incidents including accidents and complaints and ensure lessons learned shared and improvements implemented.
- Carry out regular audits of clinical effectiveness to maintain clinical standards
- Staff Management
- Provide effective operational management and clinical leadership to the nursing team within CICC
- In liaison with Matron for Intermediate Care, ensure objectives for the unit are shared and understood by the whole team. .
- Be directly responsible for the recruitment and retention of Registered nurses and health care assistants for CICC together with the Matron for CICC
- Ensure the provision of orientation/in-house training and support to new members of staff to the unit.
- Be responsible for the day-to-day management of staff, including authorising sickness / annual leave / study leave.
- Undertaking supervision and appraisals as identified by a Supervision and Appraisal Tree.
- Ensure all staff undertake mandatory training as identified as well as equipment competencies.
- Work in conjunction with HR to deal with any disciplinary / grievance issues related to staff who are directly accountable to you.
- Undertake the allocation/prioritisation of the daily workload and organisation of rota systems to ensure effective staffing levels are available for the provision of high quality care.
- Participate and lead the MDT decision-making process ensuring whole team involvement.
- Identify unmet need from a clinical perspective across the unit and highlight these needs to ensure the most appropriate actions.
- Participate in Intermediate Care and Directorate Meeting, initiating and discussing new issues and or current problems.
- Ensure partnership working with other services both in and outside of Intermediate Care
- Be responsible for the collection and collation of evidence to support CQC compliance.
- Financial Management
- Ensure the maintenance of stock levels of appropriate consumables needed with the unit
- Support the Matron for CICC regarding the monitoring of the units budget.
- Recruit staff in line with the skill mix prepared for the unit in line with the budget.
- Book bank and agency staff in line with agreed numbers of staff per shift or in extreme circumstances being mindful of both the budget and the agency cap
- Communication and Working Relationships
- Communicate effectively with a wide range of people both internal and external to the organisation including colleagues, other staff across EPUT, NHS Acute Trusts, ICS, the Community Collaborative, Equipment providers, Staffing Agencies and Social Care Colleagues
- Ensure effective communication occurs with relative, carers and visitors especially on admission and prior to discharge promoting an open and transparent service.
- Act as key point of contact for enquiries related to CICC, finding a resolution for queries and problems as necessary
- Liaise with Matron for Intermediate Care and the Head of Intermediate Care regarding co-ordination of annual leave.
- Information Systems
- Maintain the use of computerised databases and other information systems, attending training sessions for these and other systems as required.
- To be responsible for the use of these information systems to support the collection and collation of Intermediate Care data
- To be responsible for ensuring data for the clinical performance needs are collected as part of the clinical records
- Ensuring health roster is updated for all clinical staff within the unit to ensure correct payment is possible
- Ensure training tracker and supervision tracker is monitored for clinical staff highlighting areas of concern to ensure the best compliance
- Use data reports to support the provision of excellent care and ensure speedy amendments are made if standards are identified as slipping
- Professional Knowledge and Experience
- Have extensive knowledge of operational processes to ensure effective time management is in use to deliver the service in accordance to needs
- Understanding of physical impairment and how this impacts on independent living.
- Act as a source of professional advice for both the nursing team and the wider MDT
- Ensure student nurses are supported with their education and learning when allocated to the unit
- Participate in the supervision and appraisal process personally
- Ensure all mandatary training is maintained
- Be responsible for updating own knowledge and personal development
- Willingness to undertake professional development, updating knowledge and skills in line with this role, when identified via the appraisal process.
- Ensure all expectation of the NMC Revalidation process are maintained and Nursing registration is in place at all times
- Inform Matron for CICC and Head of Intermediate Care regarding all breaches of nursing profession conduct or any other disciplinary issue of any member of staff within the team.
