SEDS are launching an innovative new Inpatient Transition Pathway (ITP) and looking for passionate and skilled new team members. The ITP aims to improve patient experience, avoid inpatient stays and ensure seamless transitions between inpatient and community care. We’re now recruiting a Band 7 Clinical Specialist (Nurse or Occupational Therapist). In this role, you will:
- Provide skilled liaison between community and inpatient services across the South East.
- Support patients through their entire inpatient journey — from preparing for admission, to in-reach during treatment, to early and well-planned discharge.
- Carry a caseload and deliver evidence-based interventions that begin pre-discharge and continue in the community.
- Work closely with multidisciplinary teams and contribute to care planning meetings.

Key responsibilities of the role:
You will form strong working relationships with the South East specialist eating disorder inpatient teams.
· You will work collaboratively with the client, carers and MDT to enhance community care and avoid inpatient admission where possible.
- Assess and support patients’ physical, psychological and social needs throughout their care.
· You will be central to preparing patients for admission to Specialist Eating Disorder Units (SEDUs) either directly or supporting team members.
· You will liaise with SEDUs and the Provider Collaborative to share the co-produced goals for the inpatient phase of treatment.
· You will lead on maintaining clinical oversight of all patients on SEDU's through your in-reach work
· You will hold a small complex clinical caseload and deliver evidence-based treatments whilst promoting patients' physical health and social integration.
· You will at times work autonomously with the support of your professional lead.
· You will work closely with the wider team and chair meetings.

REQUIREMENTS
Successful applications will be required to have:
A degree/ diploma in Nursing (Both general and mental health nurses are considered for this role) Or a Degree/ Masters in Occupational Therapy.
· Current registration with the NMC as a registered nurse or Health and Care Professions Council (HCPC) as an Occupational Therapist
· Post graduate experience demonstrating ability to work in challenging situations
· Demonstratable knowledge and empathy of mental health and/or eating disorders
