As a participant on the band 5 Occupational Therapy rotation scheme postholders will rotate every 6 months through a variety of community services. These services work across the borough of Bromley providing assessment to intervention to residents living in their own homes and care homes, as well as those admitted to the inpatient rehabilitation unit.
You will be expected to:
- Contribute to the clinical delivery of the service, including completing patient assessments, designing care and rehabilitation programmes, delivering rehabilitation and effective discharge planning.
- Work in partnership with acute care and social care
- Contribute to the development of support staff within the service through training and supervision.
- Contribute to the implementation and monitoring of any existing and new patient pathways
- Maintain and develop own professional expertise as a clinician.
- Actively ensure the implementation of and adherence to all issues associated with clinical governance, policies and procedures of Bromley Healthcare and relevant professional standards (e.g. Royal College of Occupational Therapists)

Detailed job description and main responsibilities
To adhere to the Principles of Compassion in Practice, and ensure that the culture of the service is enabling for patient’s independence.
To carry out assessment of patients as an autonomous practitioner, using clinical reasoning skills and knowledge of evidence based practice to formulate appropriate care/treatment plans, utilising a range of treatment skills and options.
To use assessment findings to design patient care nad rehabilitation programmes which may be implemented by other members of the multidisciplinary team.
To maximise the functional ability and occupational performance of an individual by developing realistic treatment plans and goals on admission and discharge, and through discussion with the patient, senior staff and the multidisciplinary team, develop an awareness of patient prognosis.
To delegate appropriate tasks effectively to Therapy/Rehab assistants.
To appreciate the need for patient privacy and dignity at all times.
To reassess patients in order to progress and/or modify care plans appropriately and effectively.
To show respect to patients own beliefs, values and differing cultural needs.
To manage patient discharges effectively
To motivate and engage patients and, where appropriate their families/carers in the patient’s rehabilitation.
To participate in the education of other staff, patients, families and carers as appropriate.
To consider the needs of carers to ensure necessary support systems are in place and to involve them in their goals and needs
To participate in collecting standardised measures of outcome as required by the service.
To be aware of all issues relating to Clinical Governance and to meet standards as required.
To maintain clinical and care records for own interventions in line with organisational and legal requirements.
To manage risk within own caseload at all times, completing risk assessments and ensuring knowledge of and compliance with organisational and professional guidelines.
To carry out a full range Occupational Therapy assessment and interventions appropriate to the caseload.

Salary is determined by NHS experience and current banding/pay point. External applicants will begin at the entry point of the salary scale in line with NHS terms and conditions.
