Job overview
This post will offer the successful candidate an exciting opportunity to become part of a multi-agency service for children with Complex Needs Team, offering a unique opportunity to work within a co-located multi-agency setting fostering partnership and collaboration between children, young people, their families and other professionals. We work very closely with the Learning Disability Child Adolescents Mental Health Service and the Children’s Intensive Behaviour Support Service and there are opportunities for joint working.
We are looking for an innovative and motivated person who has a Registered Nurse Qualification in Learning Disabilities with experience of working in a multi-agency environment to join our Children Learning Disabilities Nursing Team. The successful candidate will work with children and young people who have learning disabilities, complex health needs and/or challenging behavior. Therefore you will need good observational, assessment and analytical skills and have previous experience of working in community or residential settings.
The applicant should have working knowledge and experience of using evidence based assessment tools and an interest in continence, health initiatives and positive behavior support.
CLDNT offers specialist placement for Undergraduate LD Nurses. There will be an opportunity for the successful candidate to supervise students on placement.

Key Responsibilities – Professional/Clinical
1. To provide a service for children and young people from 0 – 18 years with significant learning disabilities.
2. Works with an allocated defined caseload, working independently in a responsible, innovative and flexible manner.
3. To assess the heath needs of children and young people with significant learning disabilities, plan, monitor, evaluate and review care packages.
4. To assess and formulate individual positive behavioural support plans for children and young people whose behaviour challenges
5. To promote inclusion and raise awareness of health care needs and facilitate access to primary/health care services.
6. To comply with policies and guidelines as directed within the Central Manchester University Hospitals NHS Foundation Trust.
7. To take responsibility for a defined caseload assessing: Health / Challenging Behaviour needs.
8. To develop, implement and evaluate health Care / Positive Behaviour Support Plan packages.
9. To carry out joint assessments where appropriate with professionals from other services.
10. Skills and knowledge in the use of Nursing and Specialist assessment, e.g., Health Action Planning, Continence Advice, General Behavioural Issues, Epilepsy, Chronic health conditions and Health checks.

- RNLD / Professional Studies in Nursing and Social Work (Learning Disabilities) OR Integrated Practice in Learning Disabilities Nursing and Social Work BSC HONS OR BSC HONS Nursing (Learning Disabilities)
Desirable criteria
- Evidence of relevant post graduate training
Experience
Essential criteria
- Experience working with adults and/or children with learning disabilities within a residential and/or a community settings.
- Able to apply evidence based practice to all areas of work
- Clinical experience of working with children /adults who may have additional needs; other than Learning Disabilities i.e. complex health needs, continence, epilepsy, life limiting conditions
Desirable criteria
- Experience in working in a multi-disciplinary environment across health and social care settings
- Experience in conflict resolution i.e. positive handling techniques, de-escalation techniques etc.
Skills
Essential criteria
- Basic understanding of computers and their applications
- To be able to present information in a clear and effective manner
- To comprehend and work within the Trust’s policies of data protection, confidentiality, equal opportunities and Health and Safety to meet the differing needs of children and young people
- To be able to work within highly complex and sensitive situations which mean entering difficult stressful family settings where family breakdown/crisis often occurs.
- Have the ability to travel; the post involves community work, assessing/supporting children/young people within the Trafford area. Use of a car or access to a means of mobility to travel across the Trust footprint
