Evelina London cares for local children in Lambeth and Southwark and provides specialist services across south east England including cardiac, renal and critical care services. We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it. Our community services include health visiting, school nursing and support for families of children with long-term conditions.
Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients.

Job overview
There is an exciting secondment/fixed term opportunity to join the well established Addictions Care Team at Guys and St Thomas Hospital as Senior Addictions Clinical Nurse Specialist/ Clinical Care Coordinator for the Addiction Clinical Care Suite (ACCS).
The ACCS admits patients requiring medically assisted withdrawal / stabilisation from drugs and / or alcohol. This is a 7 day/24hr service.
This role has been developed to ensure the smooth co-ordination of care delivered to patients admitted to the pathway and ensure that they are supported throughout their journey with appropriate input from key stakeholders.
The role of senior CNS/Clinical Care Coordinator is open to registered Nurses (RGN/RMN) with a strong background/interest in Addictions, homeless health and mental health care delivery. The cns/clinical coordinator will be responsible for the delivery of evidence based care and the effective, efficient flow of the pathway including bed management.
The post holder will be expected to provide professional leadership within the clinical area and provide direction, and inspirational leadership to all members of the multi-disciplinary team working with the ACCS. The post holder will also be involved in the development of protocols / guidelines and patient information.

Detailed job description and main responsibilities
In partnership with members of the multidisciplinary nursing team the post holder will develop, implement and evaluate a seamless specialist addictions service, ensuring that patients receive the highest standard of clinical care. The post holder will act as a service coordinator and will demonstrate a high level of expertise within the specialist Addictions service, providing specialist advice, education and support to health care professionals and patients. The post holder will actively undertake clinical audits and research within the specialist area.
The post holder will be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service, including development of policies, protocols and guidelines and contribute to service developments in collaboration with multidisciplinary colleagues. Also ensures the safety and well being of services users, and always working in a manner that promotes dignity and human rights through the adoption of person centred care principles.

Education and Experience
Essential criteria
- BSC/1st degree (health related) or equivalent qualification/training and experience.
- Sound clinical experience in care coordination and management of patients with mental health and substance use and misuse needs
Desirable criteria
- Professional knowledge acquired at degree level will be supplemented by specialist training, experience and short courses to Masters level equivalent.
Previous Experience
Essential criteria
- Extensive experience of working autonomously at an advanced level within the specialist area delivering effective patient focused care.
- Experience of delivering change management both personally and as a facilitator.
- Sound knowledge and understanding of mental health, addictions, treatment and interventions
- IT literate: competent using Microsoft office and able to report clinical activity data on patient information systems.
