Job overview
This is a secondment post for 4 months ending on 31/03/2026 due to funding.
This is a unique opportunity for a skilled, experienced and dynamic health professional to lead on the implementation of a new Primary Care Capabilities Framework for Inclusion Health, Prison and Custody Suites.
You will work at a national level with a wide range of stakeholders under the direction of the Consultant in Public Health Medicine as part of a multi-skilled team. Your work and the expertise you will bring to the team, will drive the implementation of the capabilities framework across Wales. You will have substantial experience and knowledge of working in inclusion health in the community and/or in prisons and have clinical credibility in those areas.
You will deliver this bespoke piece of work within a challenging timeframe with a high level of intellectual rigour, utilising robust negotiation and motivation skills, a flexible approach, pragmatism and time management skills.
You will collaborate with a broad range of stakeholders and senior leadership, including staff working within Health Boards, prisons, primary care organisations, clinical professional groups and bodies and other key stakeholder organisations.

You will be part of the Primary Care Division in Public Health Wales, led by the Director of Primary Care / Programme Director for the Strategic Programme for Primary Care who is supported by public health consultants and practitioners, working collaboratively with the Strategic Programme for Primary Care, health boards, and the Welsh Government.
Formulate and coordinate complex, multi-stranded implementation plans for the national roll-out of the Inclusion Health Capabilities Framework with a high level of intellectual rigour. This includes strategic planning, managing interdependencies across multi-agency programmes, and re-prioritising resources to ensure timely and effective delivery, considering local and organisational needs, national policy and strategy directions
Lead the development and implementation of national policy and service improvement initiatives related to inclusion health
Interpret national policy and translate it into actionable local strategies, ensuring alignment with NHS Wales objectives and contributing to service transformation
Translate strategic guidance into service development plans, contribute subject matter expertise to shape the direction and implementation of inclusion health initiatives.

Detailed job description and main responsibilities
Formulate and coordinate complex, multi-stranded implementation plans for the national roll-out of the Inclusion Health Capabilities Framework with a high level of intellectual rigour. This includes strategic planning, managing interdependencies across multi-agency programmes, and re-prioritising resources to ensure timely and effective delivery, considering local and organisational needs, national policy and strategy directions
Exercise professional judgement and autonomy in leading bespoke, time-bound national programmes, working within broad organisational policies and reporting to the Consultant in Public Health
Apply advanced planning and organisational skills to manage competing priorities, mitigate risks, and ensure optimal use of information and other resources, responding flexibly to emerging challenges, adapting plans and timelines to maintain delivery momentum.
Communicate, influence, negotiate and collaborate effectively with a broad range of stakeholders and senior leadership, including staff working within Health Boards/Trusts; clinical professional groups and bodies; and other key stakeholder organisations.
Network extensively within their sphere to capture information and views, which will influence and shape the implementation of the framework
Lead the development and implementation of national policy and service improvement initiatives related to inclusion health.
Chairing, leading, and facilitating a range of senior level meetings, networks and conversations, managing resistance, and negotiating across systems to support the national roll out of the capability’s framework
Interpret national policy and translate it into actionable local strategies, ensuring alignment with NHS Wales objectives and contributing to service transformation.
Translate strategic guidance into service development plans, contribute subject matter expertise to shape the direction and implementation of inclusion health initiatives.
Co-design and deliver professional educational programmes, career pathways or specialist frameworks programmes aligned with national priorities.
Participate in meetings which require communication with a variety of audiences and mixtures of attendees. These may include working across the Division, across Public Health Wales and with strategic partners including Health Education and Improvement Wales, Welsh Government, the Strategic Programme for Primary Care, Health Boards and Primary and Community care colleagues, clinical senior and frontline staff across health care professional groups, health professionals in inclusion health settings such as prison, other health and employers and other key stakeholders.
Work closely with the National Steering Group for the Capabilities Framework, to evaluate the framework’s implementation, including the interpretation of complex data, stakeholder insights, and literature review.
Apply clinical and theoretical knowledge to identify best practices and contribute to continuous improvement through evidence-based recommendations and applied research, reporting progress and adapting outputs to the need.
Ensure the work is based on sound public health principles informed by the interpretation of complex data and evidence, focused on prevention, early intervention and reducing health inequalities.
Build in considerations for the accreditation of the framework
Disseminates findings to inform national learning and promote evidence-based practice
Represent work in a variety of platforms and provide updates to senior team
Whilst the postholder will not have direct line management responsibilities, they will be expected to provide leadership and direction to project support staff, ensuring adherence to agreed milestones and communicating progress.

The salary scale shown is for a full-time member of staff, with the amount being adjusted pro rata for those working less than 37.5 hours per week.
In general, anyone joining our Trust who has not previously worked in the NHS will, by default, start on the first point of the advertised pay band. Pay progression will then be determined by annual Performance and Development Reviews in accordance with the NHS Wales Pay Progression Policy. Essential criteria
- Educated to master’s level in relevant subject, or equivalent level
- Professional registration with NMC or one of the 13 AHP HCPC registered professions in Wales
- Extensive clinical and theoretical knowledge of health inclusion groups and their specific health and social needs
- Extensive knowledge of health inclusion settings where healthcare may be delivered such as specialised services or prisons
- Understanding of the relevant Welsh NHS structures and Prison Health in Wales and the strategic direction for NHS Wales
- Working knowledge of policy, strategy and guidance on health and wellbeing, health inequalities and health inclusion in Wales
- Knowledge and understanding of key stakeholders relevant to this work
Desirable criteria
- Post graduate educational qualifications in health inclusion
- Members of the inclusion health groups, or networks.

- Experience of leading on bespoke and time bound national pieces of work within primary/community care and inclusion health settings
