Job overview
An exciting opportunity has arisen for the post of a Community Support Worker in the Middlesbrough Treatment and Intervention Team based at Parkside Mental Health Resource Centre, Middlesbrough. The service is open 09.00 - 17.00 hrs Monday- Friday.
The post holder will work as part of the multi-disciplinary team delivering high quality patient focused care in a community setting.
The role will involve improving the lives of people with complex mental health disorders by supporting a team of professionals in the treatment, intervention, and management of their identified mental health needs through the delivery of excellent services to promote recovery and wellbeing.
You must have the Right to Work in the UK in order to be successfully appointed to this role. Please note, this role does not meet the required eligibility criteria for sponsorship under the Skilled Worker visa and therefore the Trust is unable to offer sponsorship for this particular role.
Advert
To be compassionate in meeting the needs of patients and their carers.
Maintain safety, privacy and dignity of all patients in the delivery of patient centred care, recognising and respecting differences including spiritual and cultural beliefs.
Responsible for recognising the signs of patient harm, abuse or neglect, and follows correct safeguarding policies
Undertakes delegated activities within level of own competency, and with due regard to risk assessment, to assist with the implementation of intervention plans as part of the multidisciplinary team.
Completes patient assessment tools and accurately summarises within patient care records as directed.
Identifies changes in patients’ presentation and acts, reports and records appropriately.
Accompany and provide support to patients in a variety of community settings, as delegated and dependent on current risk assessment.
Assists in the provision of health education and advice on health promotion to patients and their carers.
Where appropriate and with authorisation, acts as an advocate for patients/carers. To promote at all times a positive image of people with mental ill health.

The aim is to aid individuals to achieve the highest level of independence and recovery possible reflective of their situation and bring about change that is positive and improves a person’s quality of life. You will be offered regular supervision and participate in team developments. We utilise Huddles, cells, Daily Decision Meetings and complex case discussion all with the aim of promoting recovery and delivering excellent quality care in addition to supporting the team in MDT decision making.

Detailed job description and main responsibilities
This post is not open to trainees. The successful candidate must have completed the Diploma in Health and Social Care Level 2 or equivalent. Candidates must also have Literacy and Numeracy level 2 or equivalent and must complete the Trust Healthcare Assistant competencies within their probationary period (first 6 months in role).

Trust Welcome Day
Here at Tees, Esk and Wear Valleys Foundation NHS Trust, we want you to have a positive experience when you start your new role with us. To ensure you have great welcome, for all new starters joining the Trust, we offer two start dates per month on the 1st and 3rd Mondays of that month.
