Are you an acute children’s nurse who is looking to branch out into community nursing? Or a Children’s Community nurse looking for more acute-focused nursing care at home?
Bromley Healthcare, in collaboration with King’s College Hospital NHS Foundation Trust, has been commissioned by the Bromley CCG to deliver an innovative and exciting Paediatric Hospital@Home service. The team visits acutely unwell children in the community to offer treatment, preventing them from being admitted to hospital and/or staying in longer than needed. The H@H team forms part of the Pan London H@H forum with its additional support and links that will help to provide a cohesive and high quality service to the children of Bromley. The H@H will sit alongside the Bromley Children’s Community Nursing Team for support and work closely with Princess Royal University Hospital for clinical supervision and consultant oversight for patients within the service.
The successful candidate will have excellent team working and decision making skills and put their patients first. We are looking for experienced and highly skilled children’s nurses who can work autonomously to care for acutely unwell children at home.

Detailed job description and main responsibilities
This role is to work within a team of highly qualified nurses to continue to develop the Children and Young People's Hospital@Home service for the Bromley borough. This service is an acute, rapid response service to facilitate early discharge and prevent hospital admissions so these children and young people can be cared for within their own home environment for administration of IV antibiotics and review of acute episodes of illness – i.e. exacerbation of wheeze, gastroenteritis.
The team must work closely with the referring hospital to ensure appropriate referrals are received in a timely manner and the smooth transition of care from the hospital to the community. They will liaise with the hospital team throughout the time the child or young person is on the caseload to ensure the best treatment and care is provided.
