We’re looking for a Complex Case Pathway Coordinator to play a pivotal role in improving outcomes for adults experiencing self-neglect, particularly where non-engagement with health interventions is a key factor.
This post recognises the complexity of coordinating multi-agency responses and aims to strengthen collaboration between health partners across Bromley. You’ll work closely with GPs, primary care teams, and other professionals to ensure that those at risk of self-neglect receive the right support, at the right time, from the right services.
Based within the Bromley Healthcare Safeguarding Service and line-managed within the Adult Safeguarding Team, you’ll act as a bridge between agencies — facilitating communication, arranging and minuting meetings, developing effective pathways, and helping to shape a more coordinated approach to care

The Complex Case Pathway Coordinator will lead in the coordination of self-neglect cases, which are being managed through the Complex Case Pathway/ Framework.
The role includes completing all administrative, information and organisational duties to ensure maximum outcomes from Complex Case Pathway meetings. The post-holder will be highly conscientious, organised, with excellent communication, planning and people skills in order to effectively manage the main duties and responsibilities set out below.

The post holder will:

Coordinate self-neglect cases managed through the application of the Complex Case Pathway/ Framework.

Act as a point of contact, proactively developing strong working relationships with key stakeholders across the local health and social care system, signposting to the Complex Case Pathway.

Support the first agency identifying self-neglect, raising awareness of the most appropriate and proportionate options available to respond to such cases.

Facilitate Complex Case Pathway meetings, demonstrating excellent planning, communication, and engagement. This includes scheduling all Complex Case Pathway Meetings; managing agenda items, circulating the relevant papers/ information to MDT members in advance, writing and circulating minutes.

Collate and disseminate learning and multi-agency action plans arising from self-neglect cases raised through the pathway, to a high standard and in a timely manner.

Share information in line with legislation, statutory guidance and local protocols.

Support the making of onward referrals as appropriate, thereby improving patient care and outcomes.

Utilise strong communication skills, in line with the target audience (patients/ stakeholders/ board level/ large groups) and as appropriate when managing sensitive, difficult and distressing information/ situations.

Contribute to the design and delivery of awareness training across the local health system, improving the confidence of staff to deal with patients displaying self-neglect in a safe and empathetic way.

Utilise sound knowledge of the Mental Capacity Act, ensuring its principles are applied with an appropriate balance between managing risk and protection from harm.

Act as a point of escalation should barriers in health and social care systems be encountered that cannot be resolved at local level.

Responsible for the collation, analysis, validation and on-going reporting of data and performance reports, presenting data in various formats, to satisfy both internal and external requirements.

Use analytical skills to identify data quality concerns and use professional judgement to determine ways to address these, escalating such matters when appropriate.

At contract end period produce a closing report including trends, issues, lessons learnt and place-based recommendations for the improvement in the multi-agency response to self-neglect cases.

Monitor and improve the standard of service delivery via the implementation of clinical governance procedures such as audit and the development of protocols.

Identify and implement improvements to business support and routine reporting arrangements, ensuring consistency, efficiency, and effectiveness.

Develop and/ or input into business plans, securing the necessary information to inform transformation and sustainable change, supporting the ongoing improvement of service delivery for patients who self-neglect.

Have a strong knowledge of IT systems, including EMIS and Radar, to enable patient research, documentation and successful reporting.

Ensure all recorded information is accurate, up to date, factual and in line with current entry coding guidance.

Undertake duties in an autonomous manner, with tasks prioritised and advice sought from the relevant professional lead as appropriate.

Work using discretion, judgement and critical analysis skills for example, managing emails and taking appropriate action.

Manage different Complex Case Pathway cases concurrently, safely and effectively.

Travel borough wide, as necessary.
