An exciting opportunity has arisen for a dynamic, motivated individual to join our well established supportive transplant team. The successful candidate with play a key role in supporting patients requiring assessment for abdominal transplantation , meeting complex educational and emotional needs. Ensuring patients remain supported and well informed through out the process.
If you are passionate about organ transplantation and really want to make a difference to the transplant pathway for patients then we would be delighted to hear from you.
Depending on experience the aim would be for the Band 6 to progress to a Band 7 within 12 months of completing the competencies of the post.

As transplant co ordinator you will play a key role in supporting patients through all aspects of the transplant pathway. This will involve working very closely with but local , regional and national colleagues. The ability to develop close professional working relationships is an important aspect of this role
You will work closely with the wider MDT ensuring you are fully informed of all the patients needs and also being able to identify any barriers to transplantation
Patient education is a vital component of this role and the ability to work closely with patients are carers, assessing their own knowledge in realtion to the impact of transplantation
Excellent communication and organisational skills are vital for this role, the transplant pathway can be very complex and as such the successful candidate must have the ability to coordinate all aspects of care and be the key point of contact for patients
As part of this role there is an expectation that you will , on a rotational basis, provide 24 hr 7 day a week on call commitment to the service
