Our members consist of 37 GP practices which cover a population of over 355,000 patients. At NCGPA we are committed to combining the time honoured values of list-based general practice model with the benefits of working together collaboratively.

NCGPA is a Company Limited by shares, with its member practices being equal shareholders. The company is managed by a board of 8 elected representatives from member practices and three co-opted positions representing nursing, lay and non-principal GPs.

The day to day running of the company is through a Senior Management team comprising the Chair and Secretary along with the Company’s Chief Operating Officer and a dedicated office team. 

Equipped with an awareness of the changing issues affecting our members and their workforce, we strive to respond to their requirements by developing innovative solutions that are both practice-led and patient-focused. The successful candidate will be an experienced Social Prescriber, or equivalent, with at least three years’ experience across multiple sectors. The Workforce Development Lead for Social Prescribing will play a key role in supporting the development, supervision, and sustainability of the Social Prescribers and personalised care workforce across Nottingham City Primary Care Networks (PCNs). In addition to providing supervision and professional support to SPLWs the postholder will contribute to wider workforce development by supporting other personalised care roles (such as Health and Wellbeing Coaches and Care Coordinators) and assisting PCNs to plan, embed, and develop these roles effectively.

Nottingham City General Practice Alliance is a GP federation that operates to support, connect, and represent General Practices in Nottingham city. We are the lead employer for Primary Care Network (PCN) staff, employing on behalf of our member PCNs. We are close to employing over 110 ARRS (Additional Role Re-imbursement Scheme) roles.

 

Detailed job description and main responsibilities

1. CPD supervision 

  

  • Facilitate the provision of 2-hour group supervision sessions once a month to SPLWs. 

  • Offer mentorship and guidance to other personalised care facilitators to support confidence, competence, and wellbeing. 

  • Support in the provision of continuous professional development (CPD) teaching that meets needs of workforce. 

  • Be the link between NCGPA, their management team and the individual staff member. 

  • Provide coaching / mentoring support to facilitate the development of individuals on an as needed basis.  

  • Prioritise support to promote individual’s wellbeing and signpost to their manager as needed. 

  • Support with relevant standard operation procedures. 

  • Ensure accurate and contemporaneous records of sessions are kept in line with CQC requirements. 

2. Workforce development   

 

 

  • Support PCNs to identify and address workforce development needs, ensuring alignment with both organisational, ICS and national priorities. 

  • Support in the design and delivery of induction, appraisal and development opportunities to promote consistency and quality across the personalised care team. 

  • To listen and support our current workforce to promote their retention. 

  • To promote a positive work culture that supports and develops individuals to ensure that patients get the best experience and care. 

3. Service quality  

  • To support clear and consistent standards for practice, including knowledge, skills and behaviours. 

  • To support the development of systems and monitoring that improve quality and consistency whilst reducing variation in outcomes and access standards. 

  • To support with ensuring that appropriate governance mechanisms are in place to reduce risk and improve patient safety.

Essential criteria
  • Experienced and actively practicing social prescriber or equivalent.
  • Previous experience of educating and supporting others either formally or informally.
Desirable criteria
  • Experience working as a lead or equivalent
  • Teaching or coaching qualification

  • Understands the importance of experiential learning (learning in practice).
