We are looking for a dedicated and forward-thinking senior band 6 registered nurse to join our new self-care team. If you have the ability to engage staff in supporting our patients on their self-care journey, you would be perfect for our team.

The successful candidate will have experience of community-based care, be passionate about patient's choices & their abilities to self-manage and instigate personalised care.
The candidate must possess excellent communication & interpersonal skills, be able to organise their workload effectively, work well as part of a team, whilst also being motivated to expand their knowledge base and work autonomously.
The candidate will work across the Fylde Coast, as part of a community nursing service, undertaking nursing duties in patients’ homes or clinic settings. Supporting patients towards becoming experts in their own health care interventions. To provide safe, effective, individualised care to patients and to give support to their families.
Responsibilities will include leadership and operational management of a Fylde coast community nursing team, supporting, supervising and managing staff in delivering a high-quality service. To delegate work to members of the team, taking into account scope of professional practice, and nursing skill mix.
The candidate must be willing to undertake training around health coaching, shared decision making and the personalised care agenda, actively promoting a self-care model whilst ensuring you have the core clinical skills required.
Utilising support services throughout Fylde Coast, the candidate will support the provision of personalised high-quality care, ensure better patient outcomes, influence service implementation and change.
