The post holder will actively coordinate and case manage the commissioning of Continuing Healthcare services focussing on complex cases including Mental Health, across Oldham who are primarily placed in appropriate care settings, under the Mental Health Act, or receiving community care packages under section 117 aftercare arrangements. The case load will compromise both working age adults and over 65’s
This will be undertaken by leading and coordinating key professionals across Health services, Local Authority and the Independent Sector to ensure that effective commissioning of service provision occurs and that once established, is monitored and reviewed to deliver measurable goals and outcomes for the service user and NHS Greater Manchester ICB (Oldham locality)

· The post holder will act as Individualised Commissioning Nurse/Practitioner for a defined caseload of patients whose care is either jointly or solely funded by NHS GM and the care is provided by the independent sector, private hospitals or locally commissioned providers.
· Identify and case manage any clients in a nursing home who may be eligible for NHS funding ie CHC, FNC or s117; to chair and coordinate eligibility assessments and reviews. This includes representing NHS GM in the ratification of decisions for NHS Continuing Healthcare and NHS Funded Nursing care and on duty.
· Act as Commissioning Case Manager for all individualised commissioned placements, purchased by NHS GM.
· Conduct contractual monitoring ensuring that contracts in place for individual clients in receipt of NHS funding are being adhered to as well as supporting and enabling providers to improve the quality-of-care delivery.
· Provide expert advice to other professionals commissioned to provide care in relation to NHS Individualised Commissioning.
· Jointly conduct contract performance visits to ensure contractual obligations and quality care standards are being met.

· Co-ordinate and lead multi-disciplinary meetings, including patients and family/representatives, requiring excellent verbal communication skills to ensure all appropriate information is obtained in order that appropriate decisions are made in relation to the appropriate funding stream

- Complete concise and accurate reports including reports to be presented for Court of Protection cases.
- Communicate complex and often sensitive and distressing information on a regular basis.
- Nurture key relationships and maintain internally and externally.
- Attend and contribute effectively to various meetings including safeguarding and quality of care meetings.
- Establish partnerships and networks with adult social care teams, voluntary sector and other agencies to ensure patients with complex continuing healthcare need have access to all resources available.

- Qualified to Degree level in Health or Social Care qualification
- Minimum of 3 years experience as a qualified mental health professional
- Hold a professional operational post within the required field of mental health
- Registration with a Professional body ie Nursing Midwifery Council or Social Work England
- Evidence of management of change and appropriate support of strategies in place
