Guy’s and St Thomas’ is among the UK’s busiest and most successful NHS foundation trusts. We provide a full range of hospital and community services for people in south London and as well as specialist care for patients from further afield including cancer, renal, orthopaedic, respiratory and cardiovascular services.
Guy’s is home to the largest dental school in Europe and a £160 million Cancer Centre opened in 2016. As part of our commitment to provide care closer to home, in 2017 we also opened a cancer centre and a kidney treatment centre at Queen Mary’s Hospital in Sidcup. St Thomas’ has one of the largest critical care units in the UK and one of the busiest emergency departments in London. It is also home to Evelina London Children’s Hospital.
Evelina London cares for local children in Lambeth and Southwark and provides specialist services across south east England including cardiac, renal and critical care services. We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it. Our community services include health visiting, school nursing and support for families of children with long-term conditions.
Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients.

The post holder will work as an Advanced Critical Care Practitioner (ACCP) within a consultant-led, multi-professional critical care team, providing expert clinical management and care critically unwell patients, across Adult Intensive Care and trust-wide services as required.
This post is open to qualified ACCPs who have successfully completed an MSc in Advanced Clinical Practice (Critical Care) (or equivalent) and hold Membership of the Faculty of Intensive Care Medicine (FICM). Applicants must be fully credentialed and eligible to practice independently within the ACCP scope of practice.
The ACCP will function autonomously within agreed clinical guidelines, while working closely with critical care clinical team and the responsible on-call consultant intensivist, to whom they are professionally accountable on a day-to-day basis. The post holder will participate fully in the ACCP rota, providing a high level of advanced clinical decision-making, procedural skills, and leadership within the critical care environment.
The role includes a minimum of 80% clinical commitment, with the remaining time allocated to non-clinical activities, including service development, quality improvement, education, leadership, and research. The post holder will be expected to contribute to the ongoing development and sustainability of the ACCP service, support quality improvement initiatives within the department, and develop an area of specialist interest aligned with service needs.

CLINICAL AND PATIENT CARE:
To work competently in the role of ACCP, being aware of the boundaries of the role, referring patients appropriately to other healthcare professionals as required.
To perform advanced physiological assessment of critically ill patients regardless of location (ICU/HDU/Theatres/Recovery/Wards).
To use appropriate clinical decision making to initiate appropriate management/treatment according to agreed protocols and guidelines.
To prescribe in adherence to non-medical local policy & governance structures.
To be an ALS & CALs provider.
To undertake airway management skills, both directly and indirectly supervised within the boundaries of individual skill set.
To Initiate and subsequently management mechanical ventilation, which will include monitoring blood gases and altering ventilator settings to ensure optimal ventilation.
To undertake intra-hospital transfer of the critically ill patient as deemed appropriate and within their limitations.
To undertake and organise the management of patients with tracheostomy in the Intensive Care Unit and ward areas, including initiating and managing tube changes and decannulation.
To perform procedures common to the Critical Care environment including but not limited to peripheral and central venous access, arterial puncture and cannulation, bronchoscopy and transfer of stable level 3 patients.
To work flexibly as a member of the Critical Care Team supporting the critical care MDT.
Recognise and take appropriate actions with regards to critical incidents using appropriate pathways.
To interpret and utilise complex data obtained from effective use of current invasive and non-invasive monitoring equipment initiating appropriate management.
To interpret laboratory and radiological results initiating appropriate management.
To communicate highly complex and sensitive information to patients and relatives.
To present at critical care MDT and handover.
To manage a caseload of discharged patients, follow up patients discharged from critical care, and assist with the critical care Follow-Up Clinic.
TRAINING AND EDUCATION
To maintain registration, credentialing, and good standing as an ACCP, including continued FICM Membership.
To demonstrate ongoing compliance with the national ACCP FICM Competency Framework, maintaining evidence of competence and scope of practice appropriate to the role.
To participate in regular appraisal, professional development reviews, and job planning, contributing to the identification of individual learning needs and targeted development plans.
To engage in continuing professional development, including maintenance and extension of advanced clinical skills, aligned with service requirements and personal areas of specialist interest.

EDUCATIONAL
To work collaboratively with the ACCP Lead, Critical Care Education Team, and senior clinicians to contribute to the development, delivery, and evaluation of the Critical Care education strategy.
To maintain mandatory and role-specific training relevant to advanced critical care practice (e.g. advanced life support, blood authorisation), in line with local and national requirements.
To plan, deliver, and support formal and informal education and training for critical care staff, including multidisciplinary teaching at bedside, simulation, and classroom-based sessions.
MANAGEMENT AND LEADERSHIP
Adopt a leadership style that motivates and empowers other healthcare professionals to attain high standards of patient care and professional development.
