We are one of the largest acute and community providers in the West Midlands having more than 850 beds on the New Cross site.
We also have 56 rehabilitation beds at West Park Hospital and 54 beds at Cannock Chase Hospital.
We are the largest employer in Wolverhampton, the Trust employs more than 10,000 staff, covering more than 350 different roles.

- Cannock Chase Hospital – general surgery, orthopaedics, breast surgery, urology, dermatology/plastic surgery and medical day case investigations and treatment (including endoscopy, rheumatology and dermatology)
- More than 20 community sites - community services for children and adults, walk-in centres and therapy and rehabilitation services
- Primary care - Ten GP practices have now joined us and offer extended opening hours to patients.

Do you have a keen interest in specialising in diabetes care, or are you an experienced Diabetes Specialist Nurse (DSN) looking for your next rewarding challenge?
We warmly invite DSNs, as well as Registered Nurses looking to advance in this rewarding field, to join our welcoming, innovative multidisciplinary team as a Band 6 Diabetes Specialist Nurse. Here, you'll work alongside Consultant Diabetologists, Specialist Diabetes Practitioners/Nurses, Podiatrists, Dieticians and other passionate colleagues, all focused on making a meaningful difference to patient outcomes.
You'll be primarily based at New Cross Hospital, ensuring team continuity and a strong support network, with occasional opportunities to assist at neighbouring sites when clinical needs arise, demonstrating your adaptability and commitment to excellent patient care across our wider service. Our service hours are Monday to Friday 08:30-16:30 or 09:00-17:00, excluding bank holidays.
Applicants should be Registered Nurses with at least two years' experience, ideally with expertise in either inpatient, outpatient or community diabetes care. A solid grasp of current diabetes technologies, and familiarity with key clinical guidelines. Progress towards TREND competencies is preferred.

- In conjunction with the multidisciplinary team and in line with local and national targets provide specialist evidence based nursing advice to support and facilitate high quality, individualised care to patients/clients and relatives/ carers without supervision.
- Support and deputise for senior diabetes specialist nurses. This will involve dealing with complex situations and having to take appropriate action in a variety of settings.
- Provide expert advice and education for Healthcare professionals involved in the care of people with diabetes and their carers.
- Be flexible in the approach to the clinical area and to provide cover at all sites, if required, acting as professional role model providing direct and indirect clinical care, teaching and research.
Although this post is advertised at part time (4.5 days/week) we have approval pending for an additional full time (5 days/week) post which will be interviewed for at the same time as this post.

· Incorporate health promotion and prevention into care plans, and comprehensively assess patients for risk factors and early signs of illness.
· Utilise ‘making every contact count’ as an approach to change behaviour through interactions with individuals, carers, families to support them in making positive changes to their physical and mental health and wellbeing
· Plan and co-ordinate complete episodes of care, delegating and referring to other practitioners as appropriate to optimise health outcomes and resource use.
· Safely use patient group directives (PGD)/ non-medical prescribing as appropriate
· Use professional judgement in managing care events and capture the learning from these experiences to improve patient care and service delivery.
· Work within defined boundaries of practice.
· Ensure an effective and efficient patient’s journey by planning and co-ordinating of the episode of care /caseload including the smooth transition to other settings, promoting safe and effective discharge from the service and optimising communication with interdisciplinary and interagency teams as required.
