An exciting opportunity has arisen for a Band 4 Health Inequalities Project Support Worker to join our Community Diabetes Specialist Service at Whittington Health. This is a fixed‑term post supporting our health inequalities project, which focuses on improving engagement and access to diabetes care for patients who may face challenges interacting with healthcare professionals.
In this role, the post holder will support registered healthcare professionals in delivering high‑quality diabetes care. Responsibilities will include direct patient contact, ensuring administrative tasks related to care delivery are completed, and assisting with the facilitation of group consultations as delegated by the Specialist Teams.
The post holder will also lead the administration tasks for the Health Inequalities team providing consistent high lever and comprehensive support. This includes liaising with voluntary sector organisations, organising health promotion activities, and contributing to the preparation of quarterly reports for commissioners based on agreed key performance indicators.
The successful candidate will demonstrate excellent organisational, communication, and time‑management skills, all of which are essential for this role. They will be joining a service committed to evidence‑based practice, reducing health inequalities, and continuous service improvement.

We are recruiting a Health Inequalities Project Support Worker to join Whittington Health Adult Community Services. This role supports our work to reduce diabetes‑related inequalities among African, Caribbean and Turkish communities in Haringey.
You will provide high‑quality administrative and healthcare support to the Health Inequalities team, including reviewing GP lists to identify patients for targeted support, carrying out proactive outreach, and delivering one‑to‑one follow‑up consultations by phone or virtually. You will work closely with the Care Navigator and Diabetes Specialist Team, supporting patient engagement, coordinating care, completing home visits when required, and maintaining accurate clinical records.
The postholder will help develop and deliver project plans, contribute to task‑and‑finish groups, and collect, evaluate and audit qualitative and quantitative data. You will update the Health Inequalities database, produce local audit reports, and support KPI reporting. Once a month, you will help deliver the African & Caribbean/Turkish Diabetes Group session and collect pre‑ and post‑session feedback.
You will also raise awareness of health inequalities, build links with local community groups, support group consultations, and contribute to health promotion activities across Haringey. The role includes attending weekly MDT meetings and supporting the wider Long‑Term Conditions team with caseload coordination.
