We are looking to recruit a highly motivated, forward thinking, committed and enthusiastic Nursing Associate with excellent communication skills to join our Community Diabetes team. You will be pivotal in providing care to the population of Luton working across both the Diabetes team and Adult Community Nursing team, and act as a role model to current and future trainee nursing associates.

You should have excellent clinical, written and verbal communication skills and have completed the Trainee Nursing Associates course, gaining NMC registration.

Job Purpose 

The nursing associate has a breadth of knowledge across the lifespan of patients and across the fields of nursing; providing holistic and person centred care and support for people of all ages and in a variety of settings. The nursing associate works independently under the leadership of registered nurses who work within the sphere of nursing and care.

Nursing associates are equipped with the knowledge, understanding, skills, attitudes and behaviours relevant to employment as a nursing associate and will work to a nationally recognised code of conduct.

 Main Duties and Responsibilities

1. Provide and monitor care in the community setting: 

Provide compassionate, safe and effective care and support to people in a range of care settings.

Monitor the condition and health needs of people within their care on a continual basis in partnership with people, families, and carers

Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment

Recognise and report any situations, behaviours or errors that could result in poor care outcomes 

2. Planning:

Working alongside senior health professionals in planning the work load and completing tasks on Sys1/Malinko

Participating in co coordinating patients and workload 

3. Patient Contact

Being an accountable professional

Act in the best interests of the people they care for

Put people first and provide nursing care that is person centred, safe and compassionate

Recognise and work within the limits of their competence

Be responsible for their actions. 

4. Promoting Health and Preventing Ill-Health.

Support people to improve their mental and physical health and wellbeing.

Active involvement in the prevention of and protection against disease and ill health

Engage in public health, community development, and in the reduction of health inequalities 

Communication 

To liaise as appropriate with members of the Adult Nursing team.

Develop good working relationships and liaise as appropriate with parents/ carers, and other health, social or voluntary agencies.

To communicate effectively with clients in complex situations. Communication skills of engagement, negotiation, motivation and explanation will be required to enable the management of potential barriers to communication.

To be actively involved in the communication of complex information regarding clients’ needs and any other useful information to the Adult Nursing Team, and/or other appropriate medical services (such as GP) and members of the wider Adult Services team.  

Human Resources

Actively participate in annual performance reviews providing evidence of attaining the relevant knowledge and skills required for the job.

To attend mandatory and relevant training courses and to maintain up to date clinical knowledge and skills in order to perform in the role. 

Clinical and Practice Governance

Observe and maintain strict confidentiality with regards to any information in line with the requirements of the Data Protection Act.

Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.

To actively engage in developing own practice through annual clinical observations.

The post holder must adhere to infection control policies and procedures.

The post holder must participate in clinical and other audits as required. The post holder is required to participate in relevant emergency preparedness process for their team. 

General

To be aware of the responsibilities of all employees to maintain a safe and healthy environment for patients, visitors and staff.

All post holders must adhere to the code of conduct on confidentiality and be aware of and adhere to all Trust policies and procedures.

Sustainability

It is the responsibility of all staff to minimise the Trust’s environmental impact wherever possible. This will include recycling, switching off lights, computers, monitors and equipment when not in use. Help to reduce paper waste by minimising printing/copying and reducing water usage, reporting faults and heating/cooling concerns promptly and minimising travel. Where the role includes the ordering and use of supplies or equipment the post holder will consider the environmental impact of purchases.
