South Warwickshire NHS Foundation Trust (SWFT) provides the hospital services to South Warwickshire from four hospitals:

We also deliver out-of-hospital community services to the whole of Warwickshire and beyond serving a population of more than half a million from various clinics. This includes our Community Teams in North Warwickshire.
What is a Foundation Trust?
South Warwickshire University NHS Foundation Trust was formed on 1 March 2010. A foundation trust status gives us greater freedom to manage our own affairs, and makes us more directly accountable to local people through our Members, who are drawn from the local population and staff.

Disclosure and Barring Service (DBS) Checks
Many of the roles we advertise have a requirement for a DBS check to be carried out. If you are successful for a role and it requires a DBS check the Trust will carry out the initial check and subscription to the DBS Update Service, however successful candidates will be expected to cover the associated costs . Details of this will be sent to successful candidates in their offer letter.
Probationary Period
All new staff to the Trust will be subject to a standard 6 month probationary period, the details of which will duly follow in your contract of employment.

Agile and Flexible Working
All our staff will have a work base. However the Trust operates as an Agile Working Team therefore staff may be working from home on a regular basis as required by the service. However, occasionally travel around Warwickshire and into your base will be required so access to your own transport would be helpful.
The Trust also welcome staff to work flexibly and we would welcome the conversation at interview regarding flexible working.

Responsibilities:The post holder will be responsible for actively leading capacity and demand patterns ensuring the patient experience is at the centre of all decision making.
To lead bed and clinical site management, maintaining patient flow throughout the organization 24/7. Monitor staffing levels, risk assessing and at times where necessary moving staff appropriately to support clinical skill needs on the wards. The post holder will be required to work proactively and under pressure, whilst maintaining professionalism at all times. Excellent communication skills are paramount to the success of this role, as the post holder will play a key part in maintaining operational resilience. Liaise closely with all members of multidisciplinary teams to ensure accurate up to date knowledge of patient placement and flow is achieved. Act as senior Liaise with other Trusts to appropriately repatriate patients from other hospitals back to their local authority. Deliver accurate information of the Trust status at the Trusts operational bed meetings, on all specialties, ensuring that electronic data capture of demand and capacity, against the predictor and action logs are clear, with evaluation of progress on agreed actions. Lead the establishment of systems to ensure the site teams consistent compliance with DoH SITREP reporting and monitor accordingly. Lead and support in quality and improvement initiatives on flow and discharge through the hospital. Escalating as per Trust policy potential and actual obstructions in patient flow and discharges and agree actions to problem solve with appropriate stakeholders. Ensure patients are placed in the right place in a timely manner to support adequate flow and reduce outliers. Map, as appropriate, to their specialist wards when capacity available to minimise increased length of stay. As required, support in the safe opening of escalation areas as per policy. Work with other stakeholders, leading where appropriate to support admission avoidance strategies; support identified patients to Same Day Emergency Care. Oversea patient movement from the Emergency Department to assessment areas to achieve the 4hr access targets whilst maintaining patient safety at all times. Foster and demonstrate creativity, liaising with the ward teams towards efficient patient flow and pathways. Communication Skills and Working Relationships: Establish and maintain effective communication with individuals and groups about difficult or complex matters overcoming any barriers in communication. To ensure essential information is documented by self and team members appropriately according to Trust and NMC guidelines. Participate in local, regional and national conferences as required. Attend departmental and external meetings as required; and lead as appropriate to ensure awareness and dissemination of developments. Take professional responsibility for ensuring effective communication between all service providers. Planning and Organisational Skills: To prioritise workload of themselves and others, assess performance and provide clear constructive feedback to team members.
To monitor progress of work and recognise changing priorities and implement corrective actions within own limits and informing the relevant people. To anticipate, plan and advocate for development/change and benefits whilst challenging others to take an active part in developing knowledge, ideas and work practice. To plan staff off duty Rota where required. To plan, manage, and organise the team/service adjusting plans/rosters to cover emergencies. To ensure efficient and effective use of material resources/supplies. To be responsible for the management of budgets within the service.

Demonstrate commitment to own professional development. Be an active member of the cardiac arrest team ensuring ILS is up to date with a view to undertaking ALS. Undertake advanced clinical skills and extended roles in line with professional ability, training, NMC and Trust guidelines, to provide optimum clinical care to patients. Act as an effective role model proving leadership skills, motivation and support to empower nursing staff. Line management responsibility for the Clinical Site Team, Discharge Liason Team and Enhanced Care Team. Communicate and negotiate with clinical colleagues to ensure patient care and pathway is optimised. Prepare Site Team for initial leadership and coordination when a major incident ‘standby’ or major incident declared. Respond to complaints and implement remedial action as appropriate in accordance with trust polices. To participate in the recruitment and selection process as appropriate and continually for Capacity and the Discharge Team. Participate in and carry out Human Resources investigation as per Trust policy when requested to do so. Maintain a high profile and visible leadership, monitoring areas to ensure good standards and outcomes. All staff are responsible for the continual compliance with CQC standards and outcomes. Be accountable for your own professional actions and have the skills to manage workload and prioritizing tasks as a result.
