The role of Autism Post‑Diagnostic Support Team to provide person‑centred, strengths‑based support to autistic adults following diagnosis. The post holder will work alongside individuals to build skills, confidence and self‑esteem, enabling them to live fulfilling, meaningful lives within their communities. We are a supportive small specialist team.
The Support Worker will offer practical and emotional support across a range of areas, including anxiety management, everyday living, social participation, and access to meaningful roles in education, employment, volunteering, or community life. The role aims to promote wellbeing, independence and resilience through supporting to build skills or implement reasonable adjustments to be able to meet their own meaningful goals.
The Autism Post‑Diagnostic Support Worker will support individuals in both one‑to‑one and group settings, and working collaboratively with families, carers, and partner organisations.
Through providing clear information, practical strategies and supportive guidance, you will help individuals to develop confidence and belief in their own abilities.
Please note - This role is not eligible for sponsorship unless you meet criteria by temporary exemption from current changes to immigration rules put in force on 22/07/2025. Separate provisions are applied to workers who have been sponsored and held continuously Skilled Worker visa since prior to 04/04/2024.

· To act as a positive role model to those people using our services and your colleagues, demonstrating GMMH values during the course of your work.
· To establish supportive and respectful relationships with people using our services within professional boundaries.
· To assist those people using our services to identify their strengths, personal interests and goals utilising a range of recovery tools, techniques and strategies
· To share and promote coping, self-help and self-management techniques alongside neuro-affirming discussions and practice
· To liaise with carers, other health professionals, statutory and voluntary agencies to enable the needs of people using our services.
· To accompany service users to appointments/meetings of their choice and perform a range of practical tasks, aligned to wellbeing goals and promote choice at all times.
· Work in service users’ homes, in the community and in group settings to deliver appropriate and effective interventions.
· To facilitate access to community groups and networks that enable service users to participate in community activities, in order to maximise opportunities for taking on socially valued roles and positive identity.
· Advocate for service users and assist them to gain access to service who support with essentials such as benefits, training, education, employment and housing.
· To integrate and work effectively as part of a multi-disciplinary team using own initiative, personal experience and job related training in deciding on the approach and interventions required when working with a service user in delivering support work.
· Support development and facilitation of therapeutic groupwork.
· Ensure that patient health records are maintained in accordance with national and organisational policies
· Ensure that your support work goals are integrated into the Trusts record keeping process and are reviewed on a regular basis, liaising closely with the MDT and Autism Practitioners as necessary.
· To contribute to team check in sessions, MDT Meetings, other team meetings and clinical records to ensure continuity of care for service users and to ensure their safety.
· To actively participate in regular line management and clinical supervision including reflection focused group supervision and to identify personal developmental needs.
· To raise awareness of neuro affirming language with all staff and partners by modelling positive strengths based, non discriminatory, non-jargon, trauma informed language in all areas of work.
· Support other members of the multi-disciplinary team in promoting a recovery or neuro affirming orientated environment and in identifying recovery focused activities facilitating the sharing of information/education as required.
· To participate in quality assurance measures, promoting uptake of friends and family tests, satisfaction questionnaires and other measures that review the effectiveness of service delivery. Providing genuine constructive feedback and support for continued improvement of the service.
· Motivate and encourage other service users and carers to get involved in the Trust Service User and Carer Engagement Scheme and Volunteering Scheme if they show an interest in being involved.
· Attend clinical meetings to feedback progress on recovery goals.
· To actively engage in training to support competency for the role.
To participate in delivery of awareness training for colleagues and external agencies.
