We also deliver out-of-hospital community services to the whole of Warwickshire and beyond serving a population of more than half a million from various clinics. This includes our Community Teams in North Warwickshire.
What is a Foundation Trust?
South Warwickshire University NHS Foundation Trust was formed on 1 March 2010. A foundation trust status gives us greater freedom to manage our own affairs, and makes us more directly accountable to local people through our Members, who are drawn from the local population and staff.

Disclosure and Barring Service (DBS) Checks
Many of the roles we advertise have a requirement for a DBS check to be carried out. If you are successful for a role and it requires a DBS check the Trust will carry out the initial check and subscription to the DBS Update Service, however successful candidates will be expected to cover the associated costs . Probationary Period
All new staff to the Trust will be subject to a standard 3 month probationary period, the details of which will duly follow in your contract of employment.

Agile and Flexible Working
All our staff will have a work base. However the Trust operates as an Agile Working Team therefore staff may be working from home on a regular basis as required by the service. However, occasionally travel around Warwickshire and into your base will be required so access to your own transport would be helpful.
The Trust also welcome staff to work flexibly and we would welcome the conversation at interview regarding flexible working.

An interesting and exciting opportunity has arisen for a highly motivated and enthusiastic individual to join Cancer Services at Warwick Hospital in the MDT Team.
The position offers an agile working arrangement, combining both office-based and remote work. While some days may be spent working from home, the number of remote working days will depend on the needs of the service. The successful candidate will be expected to work from the office as required. This post will require weekly Monday working hours to be 10am - 6pm.
As the facilitator of an effectively run MDT meeting, the successful candidates will play a key role in helping with the delivery of cancer targets for the trust by ensuring that the journey of each cancer patient runs smoothly and is progressed in a seamless and timely manner.
The post holder will work closely with the wider trust including clinicians, general managers, service managers, nurses, secretaries and other administrative staff.
The ideal post holder will have previous experience with working within the NHS and previous administrative experience with an understanding of cancer plans, standards and targets.

As the facilitator for multi-disciplinary cancer team meetings you will ensure that MDT meetings run effectively and that the journey of each cancer patient runs smoothly and is progressed in a seamless and timely manner.
To collate and analyse sensitive clinical information from multiple Trust data collection systems and track of each patient pathway in a timely manner identifying and escalating delays in the pathway.
Support the collection, collation and input of clinical and non-clinical audit data to meet all national standards and targets.
Prepare and maintain all related documentation ensuring treatment plans are recorded in the medical notes.

Knowledge, Skills and Experience
Organise and prepare documentation for Multi-disciplinary Team (MDT) meetings liaising with a range of professionals including Physicians, Surgeons, Pathologists, Radiologists, Oncologists, medical secretaries, nursing staff and staff from other hospitals.
A willingness to cover the work of fellow Patient Pathway Facilitator colleagues during periods of absence is required.
Ensure timely notification of meetings to MDT members. Co-ordinate team diaries and organise venues and timely circulation of essential information.
To analyse cancer waiting times data and liaise with members of the MDT and Cancer Services Collaborative Facilitator to identify potential delays in treatment.
Responsible for collating all pathology material in liaison with the relevant site lead pathologist and providing reports in preparation for meetings.

Prepare typed treatment plans as discussed at the multi-disciplinary team meetings, ensuring that the consultant has confirmed the action agreed at the meeting and a copy is inserted in the patient’s health records.
In accordance with the management plans, working with the appropriate clerical staff, facilitate the next planned appointment for the patient. Monitor performance against intention specified in the treatment plan.
Interrogate data collection systems such as Lorenzo, ICE, winscribe, Soliton and other in-house and external systems to retrieve information needed to maintain a tracking process on every two-week referral, consultant upgrade and screening referrals.
Undertake administration as required for the specific tumour site teams. There will be a variety of tasks dependent upon the needs of the individual teams and their remit on specialised cancer sites.
Assist MDT team members in obtaining research data from a variety of sources including the Internet, medical and nursing journals, newsletters etc.
Ensure the provision of a high quality service. To ensure that all Trust standards and targets are maintained and monitored to improve the quality of care to all who come into contact with services provided by South Warwickshire University NHS Foundation Trust.
Undertake such other tasks as required by the Cancer Data & Performance Manager in accordance with the grade and nature of the post.
Provide cancer waiting time information for the purpose of internal and external audit when required.
Measurable Result Areas
To attend the relevant MDT meetings and complete appropriate data collection at the meeting, in particular ensuring accurate recording of clinical decisions. To actively participate in the meeting to ensure that defined National Cancer Waiting Time targets are met.
Maintain filing systems and updates and retrieve information as necessary. Work in electronic document management systems and input information into live systems during the MDT meeting.
Establish ongoing data collection, including information for national weekly and monthly reporting requirements, and preparation of audit for presentation quarterly to ensure effective monitoring of Trust Cancer standards.
Ensure data collection is undertaken in Accordance with Data Protection Act and act as central point of contact for information retrieval on behalf of MDT.
Work with the Cancer Services Managers and Cancer Data & Performance Manager in the development of Cancer Waiting Times tracking and recording mechanisms.
Participate in ongoing service improvement and date quality monitoring, collation and formatting of data for internal performance presentation by senior staff and external peer review.
The post holder will be responsible for ordering stationary and equipment for the department.
Communications and Working Relationships
Act as “administrator “to the MDT, recording notes of discussions, communicate outcome of meetings to appropriate parties. Communicate and arrange any required actions following the MDT meeting in a timely manner.
Communicate with medical, clerical and management potentially sensitive contentious information in the progression of the patients’ pathway.
The post holder will occasionally offer non-clinical advice to patients or their relatives, either over the phone or in person.
Responsible for day to day management of the MDT trackers, including carrying out appraisals and delivering their training.
The post holder will be required to implement SOP’s within the department that have been set by others.
Feedback any comments or suggestions received from service users to all members of the multi-disciplinary team and help facilitate any changes made to improve the patient pathway.
