The Children’s Community Nursing Team delivers a broad range of services, including respite provision, duty cover, acute care (including Hospital at Home) and palliative care, with team members contributing to all aspects of the service.
Within the acute/Hospital at Home element of the role, you will undertake planned clinical assessments and interventions in line with agreed care pathways, recognising and appropriately escalating any signs of clinical deterioration. You will work closely with acute-based medical teams to support children who can be safely cared for at home. The service operates a single point of access for families and professionals; the nurse in charge triages referrals and allocates workload across localities to ensure timely and effective responses.
We provide end-of-life care within the child’s home environment. To support this, the team operates an on-call rota, activated when required. Comprehensive training and ongoing support are provided to equip you for this aspect of the role.
