South Warwickshire NHS Foundation Trust (SWFT) provides the hospital services to South Warwickshire from four hospitals:

- Leamington Spa Hospital
- Stratford Hospital
- Ellen Badger Hospital
We also deliver out-of-hospital community services to the whole of Warwickshire and beyond serving a population of more than half a million from various clinics. This includes our Community Teams in North Warwickshire.
What is a Foundation Trust?
South Warwickshire University NHS Foundation Trust was formed on 1 March 2010. A foundation trust status gives us greater freedom to manage our own affairs, and makes us more directly accountable to local people through our Members, who are drawn from the local population and staff.

Disclosure and Barring Service (DBS) Checks
Many of the roles we advertise have a requirement for a DBS check to be carried out. If you are successful for a role and it requires a DBS check the Trust will carry out the initial check and subscription to the DBS Update Service, however successful candidates will be expected to cover the associated costs . Details of this will be sent to successful candidates in their offer letter.
Probationary Period
All new staff to the Trust will be subject to a standard 6 month probationary period, the details of which will duly follow in your contract of employment.

Agile and Flexible Working
All our staff will have a work base. However the Trust operates as an Agile Working Team therefore staff may be working from home on a regular basis as required by the service. However, occasionally travel around Warwickshire and into your base will be required so access to your own transport would be helpful.
The Trust also welcome staff to work flexibly and we would welcome the conversation at interview regarding flexible working.

Detailed job description and main responsibilities
· The Community Nursery Nurse (CNN) is responsible for assisting in the identification and assessment of the health needs of both children and families.
· To undertake or participate in universal developmental reviews, including heights and weights of pre-school children as delegated by the Health Visitor.
· To work closely with the Health Visitors in the planning and delivery of health promotion activities and public health initiatives in a variety of settings.
· To carry out home visits as a lone worker when delegated by a Health visitor.
· To carry out delegated family support interventions or programmes of planned care in partnership with children and carers and other agencies under the supervision of the Health Visitor. This can include delivery of health promoting activities in homes, health care or community settings, with individuals and groups.
· To contribute to the assessment of health needs and delivery of appropriate services to meet needs of children, families and the wider community.
· To provide and promote access to information and other services, to refer as appropriate.
· To liaise and work in partnership with statutory and non-statutory agencies, and other health services colleagues.
· To work in partnership with local Family Hubs and other agencies to encourage and involve parents with programmes of support. To input in the planning and delivery of those services.
· To contribute to Early Support and act as Lead professional where appropriate.
· To utilise skills of observation and recognise any significant change in client circumstances and report back to the Health Visitor.
· To work with the team to support children, young people and families who are considered to be vulnerable or at risk. To be aware of and adhere to current approved policies and procedures of Solihull Safeguarding Children Partnership Board.
· To undertake mandatory training, and service specific training as required maintaining skills and competencies.
· To participate in regular clinical supervision, appraisals and clinical rounds with delegated Health Visitor and attend team meetings.
· To support and contribute to the development of integrated team working by modelling effective team practice and good communication skills.
· To be an advocate of service change and future development.
· To ensure that individuals within the teams are aware of and work within the Trust policies procedures.
· To develop and maintain effective and appropriate communication with staff and colleagues within the Health visiting teams, Family Hubs and Early Years Settings.
· To take reasonable steps to see that the working environment is safe, healthy and hazard free for staff and users, this includes ensuring that infection control procedures are in place.
· To be competent in the use of information technology and ensure that relevant and up to date information on clinical and service developments are provided to agreed time-scales.
· To participate in audit, research projects and/or innovations following agreed policy and protocol.
· To participate in the induction of newly appointed team members.
· To maintain accurate, legible and contemporaneous records, deal with highly sensitive information, respecting confidentiality and security at all times in accordance with trust policies for data protection.
· All incidents (and near misses) must be reported to the appropriate health visitor within the team and then recorded on the InPhase System.
