Are you a passionate, experienced nurse with a drive to develop others and shape the future workforce? We’re looking for a dynamic Practice Educator to join our team and play a vital role in strengthening clinical skills, improving patient outcomes, and supporting care closer to home
As a Practice Educator, you will design, deliver and evaluate high-quality training programmes for the primary, community and independent contractor workforce. Working alongside senior nursing leads, you’ll ensure education aligns with strategic priorities, supports service transformation, and equips staff to deliver safe, evidence-based, person-centred care.
You will empower both registered and non-registered staff by building competence, supporting professional development and promoting a culture of continuous learning and improvement.

Swydd-ddisgrifiad a phrif gyfrifoldebau manwl
The post holder will support the Senior Nurse, the Clinical Lead nurse and the Divisional Lead Nurses to ensure that nurse education and the acquisition of advanced nursing skills across the NHS, Neighbourhood Care Networks and the independent sector is aligned with the Primary Care Plan for Wales, the Divisions Integrated Medium Term Plan, the ABUHB
Nursing Strategy, Professional Development Strategies, and that patient experience, quality and patient safety is at the centre of service delivery. They will facilitate the delivery of such educational needs to ensure that the Primary Care and community nursing workforce is fit for purpose and able to deliver evidenced based quality of care within our community.
• The post holder will provide clinical skills, competence training and information sessions aimed at upskilling the registered and un-registered workforce across the Division and the independent sector, in order to reduce avoidable hospital admissions, whilst ensuring that the ethos of quality and patient safety and organisational learning is fully embraced and is an integral part of patient care.
• The focus of training and skill acquisition across the division and independent sector will be to ensure patients are
supported to remain in their own home environment, cared for by competent nurses and Health Care Support Workers.
This is in line with the Primary Care Plan, the Welsh Government plan for health and social care “A Healthier Wales” and
ABUHB “Care Closer to home” strategy. On occasions, the post holder will be expected to provide support for care home
staff to enable them to manage a complex clinical caseload until such time that nursing staff are competent to deliver that
care.
• The post holder will have a key role in delivery of the skills and competence training to support the integration agenda,
including responsibility for clinical competency assessments, whilst ensuring that the ethos of patient experience, improving quality of life, quality and patient safety and organisational learning is fully embraced and is an integral part of patient care

Responsibilities and Duties
• To plan, develop and deliver training programmes for clinical skills and professional training in response to identified
learning needs for the Primary Care, Community nursing, practice nursing workforce and independent sector registered and unregistered workforce in line with education agenda ensuring cognisant of both current and future nursing lead requirements.
• To support care home and Primary & Community Care staff to develop the knowledge and skills to deliver high quality
effective care, and reduce the numbers of avoidable hospital admissions with a priority focus on those living with long term
conditions.
• Facilitating and where appropriate delivering training to ensure that the staff are clinically competent to provide high quality
safe, person-centred care.
• To support the identification and evaluation of innovative solutions as they relate to nurse education and professional development, taking account of the conflicting demands of the service and complex facts to analyse, interpret and evaluate a range of options.
• To provide support to the senior nursing team in order to develop modern, evidence based Primary Care & Community
care education which are high quality, cost effective and responsive to community health care needs.
• To liaise with the Senior Nurses, including Independent Sector nurses and non-clinical care home managers to ensure that
practice development is targeted towards patient’s needs.
• To support the senior nurse and lead nurses in the development of the Annual Training Needs Analysis. This will include
an annual training needs analysis of community nurses, practice nurses and independent sector nurses.
• To support the Divisional key priorities and delivery actions as identified in the Integrated medium-term plan (IMTP), by
prioritising workload being receptive to identified needs and working to timescales and targets.
• To provide specialist advice and direction on all aspects of nurse education and contributing to corporate decision making
at an operational level using a high level of influencing and negotiation skills.
• To regularly review and update all Primary Care & Community induction/orientation programmes within directly managed
nursing services to ensure that they outline core responsibilities, behaviours, values and expectations of the Health Board
in accordance with service needs. This will include the responsibility to have oversight of professional induction in GP practices and nursing homes.
• To participate in the formal monitoring of standards. This may be through the instigation and carrying out of service evaluations, support the coordination of policy development and Clinical Audit to monitor standards.
• To support the Senior Nurse in the development of programmes and professional competency frameworks where none
exist.
• To contribute to meeting the learning needs of team members, sharing expertise and evaluating training undertaken.
• To act as a clinical mentor to other nurses, provide supervision and mentorship regardless of grade.
