The post holder will be able to provide expert clinical expertise, operational management, to the Virtual Wards (Hospital @Home) Model within Birmingham Community Healthcare (BCHC) NHS Foundation Trust.
The purpose of the role is to work across the system in a variety of city-wide settings but not exclusive to the community and inpatient units, and to in-reach into secondary care, to identify and assess patients, to prevent avoidable hospital admissions and facilitate early supportive discharge.
The work will be undertaken collaboratively with Secondary and Primary Care, and Community Services, including Urgent Care Response, Early Intervention Team (EICT), and Specialists Services.
The post holder will work attend integrated multidisciplinary teams (MDT’s) to provide advice and support and agreed data to support the Virtual Ward (Hospital @Home) Model.

Detailed job description and main responsibilities
1. Facilitate and empower staff development and empowerment of all team members to perform to high standards and to innovate.
2. Act promptly when there are complaints in line with Trust policy.
3. Highlight circumstances giving cause for concern in relation to patient care, health and safety and follow Trust procedure regarding the reporting of incidents.
4. Support with staff knowledge and skills to promote equality and diversity and address inequalities, both in employment and service delivery.
5. Ensure supportive staff management arrangements are in place and carry out personal development reviews for direct reports.
6. Ensure all staff in team/s have annual PDR’s resulting in specific objectives and effective personal development plans in line with the Knowledge and Skills Outline Framework.

In accordance with UK Home Office requirements, Band 2 clinical and non-clinical roles and Band 3 Non-Clinical roles are not eligible for sponsorship.
Please note that the majority of non-clinical roles (Agenda for Change Band 3 - 6) with the exception of those which appear on this list - link, will not meet the minimum requirements set by UK Visas and Immigration to sponsor candidates to work in the UK.
All non-clinical roles (Agenda for Change Bands 7 - 9) that meet the prescribed skill level (RQF 6) and salary threshold will be eligible for sponsorship.

DBS Costs
Please be aware that all new employees starting work with the Trust will be charged for the cost of their DBS check if it is required for the role. The cost will be deducted from your salary in 3 payments over a 3-month period once you start employment. By applying for this role, you are agreeing to these deductions being made. The charges for the DBS include the administration fee. The total costs are £26.40 for a standard check and £54.40 for an enhanced check. The level of check will be determined by the post you are applying for. For bank Workers the full cost of the DBS check (as detailed above) will be deducted from your pay following completion of your online mandatory training. No payment will be made for your online training until it is all complete and verified by the Temporary Staffing Team. Please note: You will not be able to work any bank shifts until you have completed your mandatory training, and your DBS is in place.
