Occupational Therapist - Home Pathway
Band 6 - £45,953 - £54,254 inc HCAS per annum
Permanent - Full time 37.5 hrs
The post holders will promote independence utilising rehabilitation and enablement interventions, working collaboratively with a vast range of key health and social care teams and services within the community, as well as voluntary and public sector providers, to ensure the successful delivery of the Home Pathway Service in Bromley.
Your role will encompass completing complex assessment and treatment plans, developing and leading education and treatment, and occupational therapy service development. The post will primarily involve working with elderly patients and their families with a focus on goal-orientated treatment planning within their home.
Band 5 Occupational Therapist with previous rehab and rotational experience are welcome to apply as this will be a supported position.
This is a static community rehab post however; there is a possibility this will become a rotational post in the future. It is expected that the job holder will work across and support all community based and in-patient therapy services, as operationally required.

The successful applicant will be supported and upskilled to gain experience in a leadership role to include the following responsibilities;
- Supervision of junior staff
- Individual caseload management and supervision of team caseload management.
- Completing return to works and managing sickness as per the trust’s policy
- Carrying out detailed and timely appraisals in keeping with KPI’s
- Co-lead on the coordination of all discharges from the Home Pathway Team.
- Lead on therapy input in the weekly consultant led MDT.
- The post holder will be expected to streamline processes to support efficiency within the Home Pathway.
- Service development; a review of current Occupational Therapy services and an innovative approach to developing new aspects of the service.

Salary is determined by NHS experience and current banding/pay point. External applicants will begin at the entry point of the salary scale in line with NHS terms and conditions.
