We are seeking a motivated and highly organised CYP Asthma Integrated Neighbourhood Team (INT) Care Co-ordinator to provide high-quality administrative and coordination support to our developing Children and Young People's Asthma Service across Bromley.
This exciting role sits within an innovative Integrated Neighbourhood Team, bringing together professionals from community health services, primary care, acute paediatrics, education, social care and voluntary sector partners to improve outcomes for children and young people living with asthma.
The Care Co-ordinator will play a key role in ensuring the smooth operational delivery of the service, supporting the CYP Asthma Clinical Nurse Specialists (CNS) and wider multidisciplinary team. Acting as a first point of contact for families, schools and professionals, the post holder will coordinate referrals, appointments, meetings and communications, ensuring children and young people receive timely, joined up and person-centred care.

The CYP Asthma INT Care Co-ordinator will provide comprehensive administrative and coordination support to the Children and Young People's Asthma Service, helping to ensure the effective day-to-day operation of the team.
The post holder will coordinate referrals, manage clinic schedules, maintain waiting lists and support communication between families, schools, primary care, acute services and community partners. They will act as a key point of contact for enquiries, ensuring information is managed and shared appropriately and in a timely manner.
Working closely with the Clinical Nurse Specialists and wider Integrated Neighbourhood Team, the Care Co-ordinator will organise meetings, prepare documentation, take minutes and support multi-agency working. The role will also contribute to data collection, audit activity, service reporting and population health initiatives, supporting the identification and follow-up of children requiring additional asthma support.
The successful candidate will provide excellent customer service to children, young people, families and professionals, helping individuals navigate services and ensuring that administrative processes support the delivery of safe, effective and integrated care.

Detailed job description and main responsibilities
Service Coordination
- Support the coordination of the Children and Young People's Asthma Integrated Neighbourhood Team across Bromley.
- Facilitate effective communication between community services, primary care, acute paediatric services, schools, social care and local authority partners.
- Manage referral processes, ensuring referrals are received, recorded and processed appropriately.
- Maintain accurate waiting lists, clinic schedules and referral tracking systems.
- Act as a first point of contact for families, carers and professionals, responding to enquiries and directing queries to the appropriate member of the team.
- Arrange multidisciplinary meetings, case discussions and reviews as required.
- Ensure timely distribution of clinic letters, asthma action plans and follow-up information.
Administrative Support
- Provide comprehensive administrative support to the CYP Asthma Clinical Nurse Specialists and wider Integrated Neighbourhood Team.
- Organise appointments, clinics, meetings and training events.
- Prepare agendas, compile meeting papers, take minutes and distribute documentation.
- Maintain accurate electronic records in accordance with organisational policies and procedures.
- Ensure compliance with information governance, confidentiality and data protection requirements.
- Support the coordination of asthma education sessions, training events and community outreach activities.
- Assist with ordering and maintaining supplies and resources required by the service.
Data, Reporting and Population Health Support
- Assist with the collection and recording of service activity data.
- Maintain databases, spreadsheets and performance monitoring systems.
- Support audits, service evaluations and quality improvement initiatives.
- Ensure clinic outcomes, patient contacts and follow-up actions are accurately recorded.
- Assist in the identification of children and young people who may require additional support using service activity data and risk stratification information.
- Contribute to the preparation of reports for internal governance and external stakeholders where required.
Communication and Family Support
- Contact families to arrange appointments and provide information about the service.
- Support children, young people and families to access appropriate services and resources.
- Liaise with schools to coordinate reviews, training sessions and information sharing.
- Communicate professionally and effectively with GP practices, hospitals, community teams, local authority services and voluntary sector partners.
- Promote a positive experience for children, young people and families through excellent customer service and responsive communication.
- Support the delivery of family-centred care by ensuring information is shared appropriately and that families remain informed throughout their care journey.
Working Within an Integrated Neighbourhood Team
- Work collaboratively with colleagues across health, education, social care and community services.
- Contribute to the delivery of integrated, neighbourhood-based care for children and young people with asthma.
