Are you looking for a new challenge? Are you organised, independent, proactive, determined and enthusiastic?
Do you like working with people and enjoy problem solving? Do you want to be part of a close knit team of Care Navigators who work autonomously and as part of a team?
We have a part time post in Ardwick and Longsight working in the community to help people in the right place, at the right time.
The successful candidates will have a key role in navigating Health, Social Care and Voluntary Community Social Enterprise (VCSE) sector systems. They will act as a vital link for community and hospital staff to ensure care is effectively managed outside of the hospital setting and where appropriate reducing the risk of unnecessary hospital admission or support hospital teams in facilitating timely and safe discharge.
The successful candidate will require confidence and diversity with a range of computer skills and will be excellent communicators. Everyday is different in Care Navigation and you’ll need to think on your feet and be prepared for any situation!
The post is band 4, 17.5 hours, flexible hours with a possibility of occasional weekend and evening working.

We work with a strengths-based approach and champion independence, joined up care and encourage bigger and wider conversations.
The post holder will act as a community liaison and coordinator to work within Neighbourhood teams to improve communication and collaboration including supportive signposting and case managing with people in the community. This will ensure access to the right support, at the right time, in the right location. The service has a large community based focus and links directly with people within their own homes and GP surgeries and collaborates with staff members in the community teams and Primary Care Networks and local Voluntary Sector.
The post holder will work alongside members of the multi-disciplinary team to enhance communication and connection leading to leading to more effective citizen care with a focus on keeping people safe, well and independent in the community. The post holder will also be able to manage their own diary and caseload autonomously.
