We are looking for an enthusiastic, knowledgeable and motivated Senior Critical Care Nurse to lead the Critical Care Outreach service at the PRUH. Exceptional interpersonal, communication, operational and organisational skills are essential to be able to demonstrate a supportive and flexible leadership approach and a proactive, responsible attitude to promote confidence in the team and the service.
You will bring with you a proven track record of outstanding leadership in your current role and demonstrable extensive clinical expertise in a Senior Critical Care role within Critical Care or Outreach. You will have an in depth understanding of critical care performance indicators, terminology, interventions and care delivery and will use this knowledge and skills to support the leadership of your own team and expert advice, and guidance for clinical practice within the ward environment.
You must have achieved a specialist course in critical care as well as a Mentorship qualification or equivalent, and have completed or be considering working towards ACP/Masters qualification.

- To manage the iMobile Team and provide visible clinical and professional Nursing Leadership to the iMobile service at the PRUH.
- Actively contribute to the support of long-term ICU discharge patients, throughout their hospital admission.
- Work with ward matrons and ward managers to improve the care for deteriorating patients across the hospital.
- Work alongside the Trust Deteriorating Patient Matron to provide reports and data to feed into the trust work streams looking into deteriorating patients i.e. trust deteriorating patient group.
- Support The Trust in achieving CQUINs and key performance targets associated with deteriorating patients and contribute to Local & National projects pertaining to the deteriorating patient
- Support the management and provision of Trust wide education for staff to improve the care of deteriorating patients and work with key stakeholders (e.g. PDNs).
- Assist in the training of nurses in theoretical knowledge, practical skills and in expanding their roles.
- Develop key performance indicators and success measures for the iMobile team.
- Risk and Governance: Work with the Patient Safety team, and Matrons to review incidents, provide feedback and strategies to reduce risk leading to patient deterioration.
- Under the professional Leadership support of the Lead Nurse for iMobile, share learning cross site and develop strategic improvement programmes for the service
- Actively be involved in audits and research as part of service development.

Main Duties and Responsibilities
- Works within agreed protocols to meet the responsibilities of the role required.
- To manage the iMobile Team and provide visible clinical and professional Nursing Leadership to the iMobile service at the PRUH
- To maintain clinical credibility by acting regularly as a clinical practitioner in own area of expertise.
- Work with speciality matrons and critical care to facilitate safe and effective patient discharges and to support the wards.
- To review all post ICU discharged patients and to support the ward team in looking after these sick and complex patients.
- To actively contribute to Critical care multidisciplinary Long stay patient meetings to ensure patient care pathway and rehabilitation package is planned and appropriate during critical care stay and following discharge from critical care as per NICE rehabilitation after critical illness guidance.
- To support respiratory ward areas with non-invasive ventilation and tracheostomy care. Support the provision of teaching and training to both nursing and medical staff related to this group of patients.
- To maintain an overview of the standards of care being delivered across level 1 areas within the Trust and to provide support and guidance to the clinical staff in the management of these patients.
- To provide critical care advice as required both internally and externally,
- To act as a coach/ mentor for relevant members of staff.
- To maintain a high clinical profile through regular ward/department quality rounds.
- Undertake invasive interventions within the scope of practice as determined by the competency framework.
- Demonstrate knowledge and experience across a range of subject areas relevant to the field of critical care.
- Critically analyses and evaluate different sources of information for the purpose of providing holistic care to the deteriorating patient.
- Apply the principles of evidence-based practice to the management of the critically ill patients.
MANAGEMENT
- Supervise and provide Leadership to the iMobile team whilst also developing the leadership capabilities and skills of others.
- Meet regularly with Senior Practice Development Nurse for Critical Care to support appropriate Educational Pathways for Team members.
- Manage appraisals, sickness, and performance for members of the iMobile team in accordance with the Trust policies and procedures
- Demonstrate multi-disciplinary teamwork, supporting medical and nursing staff.
- Formulating partnerships with other service users internally and externally to the Care Group.
- Provide Leadership and Management support to Critical Care Unit Team in the absence of unit Matron
- Lead on future development of the iMobile nursing team through workforce planning, recruitment and training.
- Working within health and safety regulations and procedures.
- Assist and support in the investigations of adverse incidents and strategies to reduce risk and promote learning,
- Establish public relations; representing the iMobile team externally and across Kings Health Partners.
- Management of the iMobile resources and budget including equipment and Roster management.
- Provide Monthly activity report to assess and evaluate the effectiveness of the iMobile service and manage change in light of findings.
- To be involved in supporting flow within the department and site, including liaison with the Critical Care Unit Matron, Critical Care Shift Nurse in charge and Clinical Site Managers to ensure optimal bed usage at all times, facilitating timely admission and discharge of patients instigating escalation policies as necessary.
- Take the lead in ensuring the provision of care is in accordance with KCH Trust’s Polices & procedures and set standards of care by implementing protocols and policies. This includes adherence to Drugs Administration policy.
- Participate in developing and reviewing guidelines, protocols and standards.
- Recognise and acting upon ethical and legal issues which arise in practice.
- Participates in appraisal and personal development reviews for nursing and medical staff.
- Ensure that the iMobile service has a sound work force plan. Participate in recruitment process for own team and within the Care Group.
- To represent the Care Group at appropriate Trust wide groups and to actively participate in the site based Matrons Meetings.
- To assist the Head of Nursing in implementing new policies and service developments within the Care Group.
- Facilitate the cascade of information ensuring awareness of Care Group, Trust and NHS initiatives, supporting staff discussion and responding to feedback.
- Ensure that the culture within the iMobile Team complies with the Trust's Equality and Diversity Policy, supporting the delivery of the Trusts Race Equality Scheme and the Trust's duty to positively promote race equality and equality of opportunity for disabled people ensuring services are responsive to the needs of equality groups.
- To ensure that all staff are fully aware of child safeguarding procedures and that strategies and plans are in place across the care group to accurate identify children and vulnerable adults at risk of abuse.
- To maintain confidentiality with regard to personal information pertaining to patients and staff.
EDUCATION AND RESEARCH
- To incorporate health education and health promotion in clinical practice.
- Undertake required training for the post and utilise local and national educational opportunities for development of the role.
- Mentor iMobile nurses within the team and support their training to develop the service
- To support the development of and delivery of localised iMobile teachings and participate in educational courses applicable to critical care skills aimed at ward level nurses in line with Intensive Care Society Guidelines.
- To contribute to the wider educational activities – ensuring attendance at and contributing to the

- Participate in multidisciplinary meetings, clinical teaching and case reviews.
- To promote and encourage the application of research and of evidence based practices.

- To participate in any research projects conducted in the clinical area as appropriate.
- To practice within Trust policies and procedures and maintain codes of professional practice/conduct at all times

SERVICE DEVELOPMENT
- In conjunction with the Critical Care Service Manager, Clinical Director and Head of Nursing, assist in service developments of iMobile Critical Care Outreach.
- Under the professional Leadership support of the Lead Nurse for iMobile, share learning cross site and develop strategic improvement programmes for the service.
- To participate and assist where appropriate with the Trust patient deteriorating committee.
PERSONAL PROFESSIONAL DEVELOPMENT
- Maintain own personal development plan and up to date clinical practice.
- To adhere to NMC Professional Code of Conduct at all times.
- To act up for Head of Nursing as required.
- To ensure clinical and administrative policies and procedures are adhered to.

- To take personal responsibility for own professional growth and to develop agreed action plan with the Head of Nursing.
